Most likely you're noticing the aversion of Secret Identity. For characters that pretend that they still have a secret identity despite using their name as a codename, see Secret Public Identity. Please note how it says that characters like Doctor Strange are not using codenames. Strange does not separate his superheroism and day-to-day life by using "made-up names".

You're probably noticing how they have odd surnames for a supposedly normal person, which is the basis for their superhero/villain identity.

I think that part is covered by Awesome McCoolname, but I'm not sure. :)
