My guess based on the timestamps is that this was the sequence of events :

1)Miin U submitted an edit at 6:28:50 PM

2)You submitted one at 6:31:45 PM

3)Miin U then hit back on their browser to return to the edit page instead of hitting the edit button again, resulting in a window that didn't get the changes you had made added to it.

The other possibility is that one of the editors had their edit window open so long that the lock expired, allowing another editor to come along and innocently open a new edit window that didn't include any of their changes. I've seen that happen before.
