I was going to say Werewolf: The Apocalypse, too. I believe the Vampire video game was actually produced, and I have books that at least advertise the Werewolf game near the back covers. Silver klaives are big-deal weapons for werewolves.
Can you specify what sorts of things you were fighting?
What other weapon options?
Specific special powers the werewolf had?
How much bigger than human was the hybrid form? In W:tA it's 9 ft tall.
Do terms like Garou, Umbra, Gaia, and Wyrm ring a bell?
EDIT: Found that ad. The W:tA game was Werewolf: the Apocalypse: The Heart of Gaia, and the associated imprints were ASC Games, Dream Forge, and Unreal Technology. Ad appears to be from 1999 or 2000.
EDIT 2: Heart of Gaia was never released.

While it nudges off in a different direction than your description (no glaives that I recall, for one thing), Lands of Lore: Guardians of Destiny sounds like a similar situation.

There was a mid-'90s PC game that got rave reviews, but had horrid sales and wasn't heard from again, far as I know.
It was a kind of action/combat game with an RPG Gabriel Knight vibe. You, the protagonist, are a werewolf who can change from human, to wolf, to hybrid form, as the game requires. You have various weapon options, but a peculiar kind of glaive was the real showstopper. Ring any bells?
