Has the first one been released as part of any official outtake compilation? If so, I think it might just qualify. Second one seems more like it would be appropriate for Throw It In or Serendipity Writes the Plot.

The first example was from the first in a set of official blooper reels that were released on VHS, and has been included as a dvd extra in later releases.

We would have to actually watch the video to be sure, but the "pancakes" example strikes us as staged, in which case it could reasonably be considered part of Mystery Science Theater 3000 rather than real life.

The door seems like a different trope entirely.

The first one is probably staged input (creative content, not Real Life example)

The "In-universe example, but just fortuitous in Real Life" is self-contradictory. The following describes something from Trivia, so it can be removed.
