In my opinion, the entry for Ax-Crazy and its mention of Kill All Humans seems to cover it. Creepy, doesn't have any guilt about murder, etc.
Straw Nihilist I think requires some expression of philosophy. They have to explain their motivations in a nihilistic, "None of this matters so we might as well destroy it all!" way. A character who just kills for laughs or to come up with deadlier killing devices doesn't necessarily have a nihilist philosophy.
You could make a case for Well-Intentioned Extremist that her motivation is to emancipate her fellow dolls, and sees only one extreme method for achieving that.

for the reference, Creepy Child mentioning is still worth to say too, as the page says:
This character isn't a human, but a Yōkai.
That being said, I'm not sure which trope does it fits regardless, but Misanthrope Supreme is another one off the list.
I think that Humans Are Bastards is still involved as Medicine's reasoning, and part of her background referenced to abandoned dolls misused by humans.

Plus, like brownies, elves, and faeries of English and Scottish folklore,they range from helpful to malevolent.
Maybe Fantastic Racism or a variant, after all.

Touhou Kaeidzuka ~ Phantasmagoria of Flower View might have a problem with defining a character's violent reaction to humans in general, and currently it is listed as Straw Nihilist but I'm not sure if that's the correct trope.
Wouldn't this description would fit more in bills of Fantastic Racism?
Or does it fit in another trope instead of either of these two?
