If it was on the site that she put the stories on, I fail to see how that’s doxxing.

The only way to "clear" an edit history would be to cut the page and remake it. But while our articles don't need to be encyclopedic about a creator's personal life, I agree with ^ that if this was publicly available information anyway, then there's no issue.

Addendum: The author has recently died, as revealed to me and a few others by someone with access to her obituary. I don't know if it's okay to mention this, or if it's, again, doxy.

The opposite is expected, if a work has an author (which is... all of them), always name them in the description.

^ I think they're asking wether it's okay to mention real name when they're writing under the username they're using on whatever site hosting the fanfic.

Again, I don’t think you’re using the word correctly, OP.

It’s fine.

If the fanfic is hosted online, use the name it's published with, meaning "By crazedwriter". If someone makes Creator.Crazedwriter, other details would be fine to be mentioned there, but for fics unrelated trivia isn't needed.

There already is a creator page: Creator.Frollo Freak.

^ Which states that both are the same person's pen names. I don't know what's the protocol for such cases, maybe it's possible to make one as the "main" name, and other as a redirect (depending on which is more well-known).

It's possible/permissable; Creator.Samuel Langhorne Clemens redirects to Mark Twain and Creator.Paul French redirects to Isaac Asimov.

The former is a real name redirecting to a pen name because that's what they are more famous for. The latter is a pen name redirecting to a real name because that's what they are more famous for.
