I don't know how to picture this that won't look more like The Berserker or General Failure. You can't link to individual examples unless they're a picture, just quote it.

How about a split image with enumerated turns that shows a Mook repeatedly executing the same attack? It could, for example, be a JRPG screenshot...

That sounds like a good idea, as long as the split parts are easily visible/recognisable in the picture.

Moved image from Suicidal Overconfidence, which now could use an image. Is this lockable, or is that not good enough?

With no context at all, it looks like something else that would count, but then also (taken from the examples):

"An effective strategy when attackers have made some progress in Team Fortress 2 is for one player on a team (often a Scout) to throw himself on the objective and die, over and over again. As long as someone keeps tapping the cart or picking up the briefcase, the attackers won't lose their progress. He's probably not doing much fighting, even, electing to go around or just run by enemies rather than risk being too late to the objective."

Yes, there are many reasons you might want to suicide, in a game where you respawn. Teleport, maybe. That example describes doing it so the objective doesn't reset (e.g. flag to the enemy base). Isn't that a flavor of Attack! Attack! Attack!? Dunno, I don't mean that automatically means it should be used or whatever, but it's pretty sweet.

I dunno most of the trope page is basically just saying "these guys attack all the time" with Mooks included and all so...

If we keep that one, we may want to make the caption line up with the frame. I'd do it now, but I don't want to waste time if we lose it.
