Guru died because he was 'killed by Frieza'. As in the trauma of Frieza's actions caused him to die prematurely (hurray for loopholes)
Krillin already died before. The Earth dragon can not revive the same person more than once.
Never answered. The time limit is one year, and the wish is vague enough with the whole 'Frieza and his men' part. But I myself doubt it. The intent of the wish was to revive everyone on Namek at least

The Dragon Balls were stone because they were used up. That always happens to the Dragon Balls during their cooldown time. When they turn back into Dragon Balls and start sparkling, you're clear to make wishes on them again.
It's just usually not easy to tell that's happening to them because they fly up into the sky and then shoot off into the far corners of the Earth, but that's what they look like when they're depowered. It also happens if their guardian dies.

At least Dead Space 1 is scarier than 2 and 3. Nappa and Vegeta already figured that out.

The requirement for tanking a planet explosion would be far, far below that. Freeza wasn't at full power when the explosion happened, that was made clear. In fact, one could say that he used all of his last bit of ki trying to kill Goku, and he tanked the explosion without the use of it; in other words, with his physical body, at a power level of essentially zero.
