50+ kills per siege in a game where consistently killing 20 per battle is already an achevement.
That, or Total War, King Edward The Ridiculously Cruel. Period.

The "kill everyone" ending for New Reno.
You have to assassinate four different crime bosses, and the odds of you needing to shoot up the entire building with them are exceedingly high.
GTA IV: I was walking across the street, minding my own business and not causing any trouble, when suddenly I get a star for no descernable reason. This pisses me off because I didn't do anything to cause the cops to want to bust me, so I say "fuck it" and blast the cop. This gets me another star and more cops to show up. I decide this is a great time to go on a killing spree. Long story short, got to 5 stars and was using an attack chopper to straif a road block on a bridge, crashed into the bridge and fell into the water below. When I climbed out of the water I used a cheat to remove my wanted level and tried to remember what I was doing before the cops decided to try to arrest me for jaywalking (which I countered with Arson and Murder).

Unreal Tournament: Facing Worlds + 15 bots on Easy + sniper rifle + full sniper rifle ammo + standing on top of my base = picking off the opposing team almost as fast as they can respawn.
Head Shot! Double Kill! Multi Kill! Ultra Kill! Mo-Mo-Monster Kill! Mo-Mo- Mo-Mo- Mo-Mo- Mo-Mo- Mo-Mo-Monster Kill! Kill. Kill...
Repeat until out of ammo, then grab more and repeat.
Quake 3 Arena: The Longest Yard + easy bots + sitting on the railgun spawn point = lots of easy snipings (and quite a few mid-air kills). On luckier occasions, I've managed to get 20 kills with 20 shots. I say luckier, because if the bots have found enough armour before I fire on them they'll survive the first hit.
Accidental mistakes are forgivable, intentional ones are not.
