She was killed by accident by Zim. Zim created some sort of energy absorbing blob that ate her. Not unlike how he killed Red and Purple in the movie actually.
There was one other Tallest between Miyuki and Red/Purple. Tallest Spork succeeded Miyuki. He was eaten by the same energy absorbing blob that ate Miyuki. IIRC, the blob returned because of some doodad Zim kept holding onto after the initial disaster.
Zim killed two Tallest with one Weird Science F*ck-Up. Again, not unlike what happened to Red and Purple....

Actually, at the rate Zim kills off his fellow Irkens, I'd be surprised if there are even that many Irkens left.

Ten Minutes was another unreleased episode where Zim's PAK gets separated from him and grafts itself to Dib. Zim has to recover the PAK in ten minutes. If he doesn't, he'll die and the PAK will overwrite Dib's mind and make him into the new Zim, with Zim's personality and memories and characteristics and whatnot.
It's some really interesting lore about Irken biology, but technically not canon since the episode was never made. So I've often wondered how the comics handle it.

The closest thing we ever got to that was the body swap issue, iirc.
Gaz became Zim, and basically took on his personality while retaining her level head and intelligence, resulting in her taking over the world. Dib became Gir, and earnestly wanted to fix things and save everyone but was rendered too insane to even remember what he was trying to do by the end of the issue. I don’t remember what happened to Zim and Gir in Dib and Gaz’ bodies - I think Gaz disposed of them in some way, but oddly the story wasn’t really about them.
"Ten Minutes to Doom" also revealed that PA Ks are only really compatible with Irkens. If they try to connect with anyone who isn't an Irken, they kill their hosts within ten minutes.
The Irken and PAK relationship seems to be a symbiosis — an Irken cannot survive without a PAK, and a PAK cannot survive without an Irken.
One has to wonder what led the Irkens to be so dependent on technology that they would do this to themselves.

Jhonen Vasquez considered it a possible ending point for Invader Zim. Zim and Gaz's effort to recover the PAK from Dib would have failed, resulting in both Zim and Dib dying. IIRC, Nickelodeon nixed this idea immediately because killing off both characters would make it impossible for the show to ever come back, should they want it to.

Until Zim inevitably flattens that place too. Then he gets all horrified and weepy when he discovers the remains of his beloved cold unfeeling robot arm.

And also on him not doing anything while being punished.
I think Red and Purple understand that their best odds are just placating him and keeping him as far away as possible.
Maybe you'd be less disappointed if you stopped expecting things to be Carmen Sandiego movies.Red and Purple are obscenely lazy and also at the same time malicious. In the very first episode they considering sending Zim to a planet of explosive glass and the planet of slaughtering rat people which would of likely killed him but chose to send Zim to Earth instead because they wanted to drag out Zim's suffering.
Ultimately that proved to be their undoing.
They didn't send Zim to Earth specifically. They didn't even realize there was a planet there at all. It was supposed to be a glorified Snipe Hunt.

The answer probably amounts to the same reason Megatron kept Starscream around despite his obvious treachery: "I'm an idiot!"
These are the same guys who refused to change direction to avoid colliding with the Florpus after all.
