I think if the KS characters were to exist in real life, it would have to come at the cost of the KS game not existing, because I would be horrified to see someone like say, Hanako, being put in a situation where she's famous and has hundreds of voyeuristic nerds constantly pursuing her.
I make video games! Or plan to, anyway.Ah, but if the KS game did not exist, but the characters in it did, you would never know about them anyway. They would just be a couple of strangers out of 6 billion+ who you are never likely to meet because they probably live in a different country and without prior knowledge you would have no reason to look for them.
If the game and the characters both existed in reality, than the characters would be personally thrust into fame, regardless of whether they want that or whether it's healthy for them.

Anyone else noticed how Shizune gets a disproportional amount of screentime in Act 1, even if you aren't making the choices that lead to her route? I noticed it my first time through, but now its especially noticeable to me since I've seen all the routes Act 1 can take.

Though I don't think that's the only reason. Sure, romantic interest is probably part of it, but it's mostly because the only routes you can get without explicitly turning her down are Emi's and her own, and because out of the three girls social enough to try to befriend Hisao on their own, Shizune is the only one who is in the same class as him and has opportunity to be around him.
Going by consensus, it depends on whether you want to understand what you're doing or not.

Either route you pick, you will encounter some of the strongest feels the game has to offer.
I started with Rin, then Hanako, then Emi, Shizune, Lilly.
My order was random, but I would suggest ordering them in order of how emotional they are. Because I did the Rin and Hanako routes first one after the other (AKA the most tearjerking routes), I was kind of "burnt out" for when I played Emi's route (which I played a day after finishing Hanako's route) and I feel as if I kind of missed out on that route a bit. You can only feel so many feels in a short period of time.

How many times do you do the girls anyway, if you have to specify "always"?
Personally I did them like Shizune (because Misha), Hanako, Lilly, Emi, Rin.

Another question, does the library allow you to pick up from a certain point then play on, or does it just play that particular scene and doesn't continue? Because while I lucked out of Emi's bad end with a timely save, having to replay is still a pain.
