I'm really not sure why "just a character looking menacing" should disqualify it from being nightmare fuel. IMO: if the character "looks menacing/angry" and the menacing or angry expression is this really uncanny grimace, then it is nightmare fuel. To be brutally honest, I occasionally find these moments miles scarier than something like an Eldritch Abomination or a generic monster. In this case it's a guy with a scythe, a displeased look and red eyes staring at the viewer set against a creepy dark background — while it could've been more effective, the color palette and general framing tells me "this is not a person you want to meet". I find it kind of unique because the effect is a lot more subtle compared to something like "raaah I'm scary because I'm a monster with too many eyes".
