The first time I watched My Goldfish is Evil as a kid, I thought it was a spin-off of Stanley because Admiral Bubbles resembles Dennis the goldfish.
Keep in mind those shows are aimed at different demographics.

Tonight I'm eating crow."Good Vibrations by The Beach Boys was one of the first times a song's Word Salad Lyrics confused me - I asked my mom what it meant because I was at the age where you expect parents to know everything, and because I figured it was something popular when she was young. She evidently didn't like the song or just felt like being a smartass, so she told me it was about orange soda, which just confused me even more. Years later I found out the joke was that they had used it in an orange soda commercial. So to use another example on that page, it would be like saying "Lust For Life" is about going on a cruise.

So you know the part in The Little Mermaid (1989) when Eric throws the flute into the sea to represent letting go of his search for the mystery girl? I, and a lot of other kids apparently, didn't get that it was symbolic at first and wondered why he threw away a perfectly good instrument.
..But my misunderstanding went even further cause the flute sounded like it went 'uh oh!' before disappearing, so I thought it was warning him about 'Vanessa'.

Not me, but my sibling. For a long time, they thought Abe from The Simpsons was gay, presumably because of a short clip on Hulu from the episode "Gorgeous Grampa" where Marge assumes he's gay after they find his old heel costume.

For quite a while, I saw the cover of Paranoid (Album) as a strange shape instead of a soldier.
I thought in Sonic the Hedgehog (SatAM) Antoine was Tails' dad because they look similar. I didn't find out until I was an adult that they aren't even the same species. EDIT: just noticed someone else on the last page thought almost this exact thing.

Yeah, that was me. I didn't think he was related to Tails, but I did used to think he was a fox, since that's what the early Archie comics said he was. I'm not sure if that's a case of someone deciding to change his species later on, though, or any possible miscommunication between the comic writers and the people in charge of the TV cartoon. I don't know when his species was first stated as "coyote" because I stopped following the comics early on (around the Endgame arc leading up to issue 50) and only watched a few episodes of the cartoon.
Although come to think of it, since coyotes are heavily associated with the American Southwest, it's ironic that he's the one with the French accent, since foxes are actually found in France but coyotes aren't.

On the misinterpreting album covers topic: The Tragic Kingdom artwork incorporates what are supposed to rotten, fly-infested oranges... but the first time I glanced at it in a record store ad I thought they were chocolate chip cookies growing leaves.
I thought Antoine's name was pronounced 'Ant-ee-own' cause I also kept misreading it as Antione.

Another one, in the game Bionic Commando, I assumed the main character had lost his arm, so they gave him the bionic arm as a replacement.

As a child, I remember seeing some movie with an ominous-looking Cymbal-Banging Monkey on the cover in the horror section of the local video store, and I always thought it was some movie about an evil monkey toy come to life. Turns out it was Monkey Shines, which doesn't have a Cymbal-Banging Monkey in it at all, but is instead about a murderous capuchin monkey.
When I first saw this
episode of Chikn Nuggit, I misunderstood how the word "by" was used in Slushi's line "Being told you're gonna get all these great things by our parents' generation" and thought she was thinking about her mother rather than imagining an older version of herself.

There was a Batman Gotham Adventures comic where Batman deduced Catwoman's presence at a crime scene by finding one of her nails.
I just realized that's what it was and not a miniature banana.
