I hate Eye Scream so much that I haven't even looked at the page for it. It makes me extremely uncomfortable, no matter how realistic or cartoony it is.

Protection and power are overrated. I think you are very wise to choose happiness and love. Current avatar: ConnieFaint in Shock, since the idea of losing consciousness gives me the creeps.
For every low there is a high.I can handle blood and small amounts of gore in most cases, but Half the Man He Used to Be (when it involves "realistic" internal organs), Eye Scream, and Agony of the Feet very often make me squeamish.

Speaking of melting into a puddle of cells, Acid Pool is also pretty horrible. The unimaginable pain of essentially corroding to death.
And I Must Scream is also horrible, of course, for obvious reasons.
And while it probably doesn't count as a trope as such, the Real Life section of You Are Already Dead freaks me out with the number of ways you could just be fucking dead and not realize it.

Any gore trope, but Eye Scream and Tears of Blood are the worst. They make me squeeze my eyes shut.

Mind Control freaked me out as a kid. Not only do you completely lose your agency, but you've become a tool for someone else. It's messed up, especially when the person is fully aware of what's happening but can't stop themselves from hurting loved ones, committing heinous acts, etc... is what I want to say. That's stuff that occurred to me later in life and I now find it a good dose of horror; as a kid, it purely boiled down to Hypnotic Eyes scaring the shit outta me.
I have a pretty bad fear of Parting-Words Regret as well as Never Got to Say Goodbye. While I haven't experienced the former, I have experienced the latter on several occasions, so it hits a bit too close to home for me.

Mine would have to be Painful Transformations, especially the slow and unwilling kind. Most horror movies and games have little effect on me but things such as SpongeBob’s transformation into a snail in “I Was A Teenage Gary” (which is likely what gave me this fear as a kid) and the transformation scene in An American Werewolf in London still unnerve me.
