Besides the opaque name of the latter, I'm having trouble seeing a distinction between them.

Apparently Careful with That Axe is when the Metal Scream is sudden, supposedly with the intention of scaring listeners.

A. That doesn't strike me as a meaningful distinction or tropeworthy on its own.
B. "With intention to scare" sounds impossible to verify without Word of God.

Oh, I always thought Careful with That Axe was just The Same, but More; something like "a metal singer screams to express emotion" vs. "a metal singer screams really loudly as though their legs were cut off", which is explicitly how CWTA describes itself.

I mostly got the "with intention to scare" distinction from Metal Scream's description:

Compare Careful with That Axe, where the singer unexpectedly screams with the intention of surprising the audience.

Careful with That Axe does mention it, though:

While some artists scream constantly, others will use unexpected bloodcurdling cries and screams to convey fear, pain, and madness, or just to freak listeners out.
