I like it. Barring anything else, I don't mind the Vampire Monarch overlap so much, since there's a lack of obvious "monarch" effect (eg. crown, throne, loyal subjects, etc)
Suggestion does not look like a vampire to me. He/she looks like some kind of moth man or something.
Health sure is versatile. It's possible to be both light-headed and dim-witted. At the same time, no less.
I think he fairly obviously looks like, uh, a bat-man.

To be honest... I can tell that the suggestion is some sort of bat monster. But it's otherwise so far away from the standard "vampire image," that I probably wouldn't recognize it as one if I saw the it outside this context. I guess since if it's on a page specifically labeled "vampire," it's probably fine. But it also doesn't really look much wealthier than the original image (maybe even less wealthy, since I can tell that it's hanging around in some cave?)

To be honest, I find the suggestion quite lacking in portraying the "rich" aspect of the trope as well. Sure, the costume looks quite fancy, but in most fantasy artwork almost every vaguely-human characters wear fancy garbs.

I think OP's suggestion portrays the "rich" part better, but your suggestion portrays the "vampire" part way better.

Ahoy gang!
The current image is a bit hard to tell she's a vampire with only the teeth to show (and they're pretty small). Here's some concept art of Vampire Monarch Lord Draken from Runescape that I think could convey both wealth and vampirism without it being too obvious that this is a Monarch. As always feedback and alternative suggestions are welcomed.
