The image is the exact same as Sonicthe Hedgehog. Even the caption is the same, and the image links to Sonic Adventure's main page, just like on the series' nightmare fuel page

Keep it on the page for the game it comes from.
I'd prefer to keep it on NightmareFuel.Sonic The Hedgehog; where it's been longer. Removed the pothole on the image on NightmareFuel.Sonic Adventure.

That could work.
I replaced my original suggestion with a slightly different, higher-quality version.
Use 4 for NightmareFuel.Sonic The Hedgehog and keep current on NightmareFuel.Sonic Adventure.
Actually, a lower-quality version of that pic was formerly on the page, but this thread voted to replace it with the current.
That was years ago, consensus can change in that time.
I think the Uncanny Valley effect of having a human head on a robotic scorpion-thing is enough to qualify. It's certainly creepier than what is basically water-Godzilla.

Pull the SA1 image. It just looks like a bog standard kaiju, and it's not even that scary.
It's scary enough to be mentioned on the page, and keep in mind this is about things that are scary by Sonic standards.
Being on the page in and of itself is not a good justification—remember that NF is chronically misused. Plus context is important and sometimes not possible to get across in an image.

For Sonic Adventure: Red Mountain would be a better candidate. Especially the lower part with all the lava and the fiery skulls.◊
For Sonic Spinball: Scorpius is pretty intimidating, (more so when angry)◊ but the lava-vomiting disembodied heads from Lava Powerhouse or the animal corpses in the roboticizer are other options to keep in mind. Man, that game went all-out for an early 1990s Sonic game.
Don't have access to You Tube or Twitch right now, so don't know how to find other screenshots of those levels. Anyone have any alternative video sites in mind that regularly host video game clips?

Reclocked. I've pulled the pic from NightmareFuel.Sonic The Hedgehog...I lean toward 16.2's take on that boss over that in 4.
Scorpius with his mouth open works for me
16.2's a lot grainier, so how about this version?
