The piano puzzle is already done, but Carl makes a mental note to play U.N Owen was Her the next time he sees a piano.

YES! That's what I was going to choose, but then I decided it would be more fun to let the players choose. The bunny ears are a double reference, to both Ruby Quest and Klonoa, since this game is about dreams.
Carl feels that he has a rabbit's ears! He is startled, and asks Grimaldi and Matilda what's happening to him. "I... don't know. I've never seen anything like this." "One thing's for sure: the sooner we get to the Queen, the better!"
"We'd better get moving and find the others. We looked around in the room we were trapped in, but we couldn't find anything useful. Let's move on."
Should they go back (to the room where Carl landed) or left (to the room where the piano keys were)?

They go back to the room where Carl landed. Nothing has changed, unless you count the relief Carl feels over no longer being alone. Not to mention Carl's bunny ears.
There are two exits here. There's the right door, which you entered through to get here, and the left door, which Carl has not taken yet (also the one that Mr. Shadow exited through). There is more to explore in both directions.

Carl once again feels his new bunny ears. They're very soft and fuzzy, and feel very nice. Despite this, Carl dejectedly wonders why this happened. He asks Matilda.
"I'm not really sure... if I didn't know better, I'd say you were actually turning into a dream creature! I don't know why, though. You and Ophelia are the first humans I've ever seen awake in this world."
"Can we keep looking for our friends? I miss Bruno..."
To keep it less confusing, from now on I will use north, south, east, and west instead of left, right, and so on.
Carl, Grimaldi, and Matilda have entered the first room through the east doorway. Should they go back and continue exploring the eastern part of the cave, or go through the west doorway?

They go back through the east door, into the room with the golden door. They go north to the room where Carl found the piano keys.
They go north again and enter a new room.
There is an exit on the east wall. Other than that, the only thing of note is a panel on the wall with three levers: One red, one green, and one blue.

Carl starts to pull the green lever. It immediately pops out of its socket.

Carl quotes Yzma from The Emperors New Groove as he pulls the other two levers. They too pop out of their sockets.
