I think it's either mental strain caused by the extra quirks interfering with brain functions (also, given that the Nomu often have exposed brains with eyeballs poking out, I think it's safe to say there is brain damage) or the strain put on the body causes so much agony that it's literally mind shattering. If it's the latter than it would explain why Gigantomachia's stamina quirk allowed him to remain cognitzant after having more quirks given to him.
For the most part, saying that someone is "mentally" ill or has "mental" problems is a misnomer. The issue is usually some chemical or physical issue within the brain itself that ends up causing symptoms we perceive as mental.
So logically, something goes wrong with the physical brain that makes a Nomuu act like a Nomuu. I take it that the brain injures itself processing quirk factors that are wildly different, and those injuries are in roughly the same place(s) where the brain regulates most of the "humanity" functions.

You know, it's obvious that Shigaraki wins over Ujiko, but we still don't know about Gigantomachia. He might still be farting around on his own, maybe trying to break out All For One by himself - he was mentioned to have showed up at Tartarus a few times.
Though maybe Ujiko can convince him to work with them, since he seems to have some influence over him.
If Deku is Mothra that means monster Bakugou is Godzilla.
Edit: Also this makes Clear Tensei wanted to be the kind of hero who helps kidsin need like Shigaraki, the coolest heroes are the ones who help kids like him in need.

Every accusation by the GOP is ALWAYS a confession.Ya know, I know its not straight up confirmed in the manga, but its still kinda silly how character page treats Doctor and Dr. Tsubasa as different characters considering that beyond looking identical, having same voice actor, they also both have gear shaped goggles even if "Ujiko's" are bit different :p

That aside I guess my theory about Shigaraki's hands being the hands of his dead family members is confirmed now. Though I didn't realise it also indirectly confirmed that his quirk doesn't work on hands until I checked this thread. That's interesting, but the hitchhiker-punching thing is out the window now.
Also I just noticed Stain is in one of the panels in the list of things Shiggy hates which is cool.

Arrgh, now I got flashbacks from "Handshakers"...
That aside, I can see where the author might be going with this...
At one point, Deku will reach out to Shigaraki in order to save him from some perilous situation with his bare hand, calling to him to take it(with zero clue that hands are safe zones from "Decay")...and after some serious hesitation,Shigaraki ends up accepting the gesture, saving him both literally and symbolically from the destructive spiral that's his life.
A hero that saves everyone.
A boy that actually was saved by a hero.
The patterns are forming towards that scenario from the look of this, in my opinion, that is. (not absolute fact.)

Well....that's pretty messed up.
Actually, taking Shiragaki's hand would be the worst choice since we know his quirk doesn't work on those. It wouldn't prove anything.
