I'd like it if they do have Deku not originally Quirkless, to have him just have a fire variant on his parents.
Not a particularly strong one, maybe something like Will O' Wisps, but Af O still took it for the purpose of combining it with dozens of other flame quirks.

Like, how in the world did they take the Quirk before Izuku or his Mom knew he had it, when would he have meet All for One, what about the whole toe-joint thing, why even bother with a convoluted scheme to take unproven Quirks from random children in the first place? All of that was just off the top of my head, and I'm sure there are other issues.

Well, considering that if Izuku had gotten a hybrid quirk from his parents that would have resulted in him breathing "fireballs" or the ability to draw fire towards himself (or just simply breathe fire or draw stuff towards him in case either quirk went dormant in favor for a dominant genetic trait) then sure, it does sound like a...
...(fridge)...
...A nomu with a "Fire Attraction" quirk, combined with "Fire Immunity", would be great to fight Endeavor with. It would even render him powerless if this nomu drew the fire away from other nomus to ensure they survived longer as well.
Even if "Fire Attraction" would be a worthless quirk on its own without the "immunity" or "resistance" towards it...it could still be a great quirk with the right combo.
Just theories of course...
I think retroactively saying Midoriya did have a quirk that was stolen would just be plain contrived, plus there's the pinky toe thing that was already brought up several times.
I’m rather hoping the whole thing is quickly put to bed by a simple scenario of the doc going over Deku’s profile & stating that he suspected something was up with him because he verified first-hand that he never had a Quirk.

This is a lie."He might try and have the LOV go after Deku for the sake of getting a genetic sample (blood alone wouldn't do) for the sake of his Noumu-research (Since Deku is one of few people who can hold and use multiple quirks without triggering the Quirk Overload which renders the noumu braindead...)
We know from the movie that OFA does show up on genetic tests as with All Might, and can be copied but not stolen, so gene samples of it for cultivation, is a legit thing in the setting...
I could see the possibility of Deku having had his quirk stolen being used by the villains as some kind of psychological warfare later on.
If i may just play Devil's Advocate for a bit:
I don't actually believe in this theory but it's fun to think about.

- when would he have meet All for One
- the doctor has a teleportation quirk that allows him to send people and objects to a person he knows well within a set distance. All For One hides out in an abandoned factory a block away from the hospital, doctor sends him Izuku, takes hik back after a certain amount of time.

- fake X Rays. Either doctored or straight-up not Izuku's.

Must admit, it is a clever scheme to get quirks for nomous or AFO without having to abduct adult street thugs or orphans which no doubt would rise suspicion.
Stealing budding quirks from kids and legally declare them "quirkless" from the get-go is much more efficient and covert.
It can be especially effective if Ujiko is considered a specialist in late quirk development/quirkless children, since it makes the potentially large swathe of quirkless children connected a single doctor less noticeable.
That said, the single biggest issue with lying or doctoring an x-ray is a simple one: What happens the next time that kid gets an x-ray?
Well, theyre not going to soecifically X-ray a kid's foot unless they injure their leg somehow, in which case. Well, the jig is up unless Ujiko has some other connections. Either that or some application of a memory-altering quirk for everyone involved.
Well, Izuku has broken his leg at at least one instance afterward,(Think Giant Robot VS SMASH) but I guess having Recovery Girl around nullifies the needs for x-rays... I presume.
Recovery Girl uses standard treatments alongside her Quirk, she presumably takes x-rays

Like I said, I'm sure it's possible to come up with an explanation for all the problems with something like this, but at this point is that really something worth doing is my bigger issue.
It's a similar situation to the "what if he can use the past users Quirks" thing that actually is going to happen, but even then I feel like that and Izuku not actually being originally being Quirkless don't really have the same kind of impact on the narrative, while execution is still the most important thing, one just feels more... "off" than the other. But that's obviously personal opinion.
On to something different - what if Izuku's Dad works for Deternerat? The prototype main character was supposed to be a super item salesman or something right? Well just recycle the concept and apply it to his father instead. Not like he's a member of the Meta Liberation Army or anything, just like a low-ranking grunt of a company, and that just happens to be the company he works for.
I think it's mentioned somewhere that the dad is a chef. One of the databooks, maybe.

And we still don't know if Spinner is one of those types who looks weird and has some unrelated power, or if his power is to do "lizard things".
Or, possibly, he just looks like a lizard and has absolutely no special abilities at all.
