Also, as The Avatar State shows, they have been fighting back, and the Fire Nation has largely been stalled for the last 100 years. The southern Water tribe has already been depopulated by repeated Fire Nation attacks, so it's not a good example.
Blind Final Fantasy 6 Let's Play
I dunno, the finale showed the Fire Nation basically obliterating the northern tribe until Aang went Avatar and wrecked them with the earth spirit. If they had attacked a year earlier, they might've taken them down successfully. And then in S2 the Fire nation sacks Omashu without much difficulty. Granted, it's because Bumi surrendered, but given that outside of Ba Sing Se (which hasn't actually appeared onscreen yet), pretty much all the earthbenders shown so far have been resistance movements fighting against a more powerful foe rather than fighting on even terms.
But on the other hand, the earth kingdom is shown to be pretty fragmented anyway. Despite the name, there doesn't seem to be much of a centralized authority, which means each individual earthbending group is fighting independently.

There is but it's more interested into holding onto its own power then actually fighting the war.
And keep in mind that Ba Sing Se has been successfully resisting attack since at least the time when Iroh was young and in the army.
Blind Final Fantasy 6 Let's PlayThe original reason for the war... actually may be a bit spoilery. But suffice to say that the Fire Nation didn't necessarily want to decimate the other nations. Just conquer them. The only reason for the Air Nomad genocide was to prevent the next Avatar from stopping them.
As for why they needed the comet so badly? Well, the Earth Kingdom had been putting up a significant resistance, so even if the Fire Nation would win in the long run, the comet's power would give them an instant win. And the sooner you're able to end the war, the better, no matter what side you're on.
