Not sexually or anything, but nobody wakes up in the morning and thinks, "you know, today I'm going to stick my face through a chunk of drywall while wearing the anime version of a nun's habit. It compliments my eyes." Gaga's smart. She knows that her ka-raaaazy fashion choices get attention, so she's playing it to the hilt.

That's just her shtick. I know people are like "she should just wear normal clothes", but every successful artist regardless of their genre has a distinct image. Lady Gaga's is intense bangs and odd clothes.

She's the only pop star (at least that I can remember) with no separation between her onstage persona and off. Cher wore weird shit to award shows, but it seems quaint now.

Some people who defend Miley's antics in this show accuse everyone who shits on her as "slut-shaming" and thus shouldn't be done. They then quote a part of "We Can't Stop" to drive the point home. Which I think is BS because I believe, slut-shaming or not, that calling out stupidity takes precedence.

then again, didn't a couple of guys from Rammstein pull off similar antics in the past?

Being serious, death and black metal are so ludicrously "controversial" that they wrap back around into noncontroversial status where they are eminently safe.
