The author state Mikasa's namesake is the warship Mikasa, who's name is written in kanji. However, Mikasa in the manga is written in kana. The official translation is Phonic!Mikasa while the scans of Fantasy Unicorn Brigade is Kanji!Mikasa. As for subs, there is no official subs. The team put fansub file on official tape (under an agreement), who used Kanji!Mikasa. There is a small paragraph on Wikipedia on this.

I think it's best to follow the official translation vetted by Kodansha. I think the English (or American) edition is done by Kodansha US division.
I think it was also Kodansha who did a beautiful bilingual edition of Card Captor Sakura.

Sasha was alright in the third episode, but a lot of the humorous moments in this one fell flat to me. I like it more when it's focusing on the drama.

They change a couple names from the official translation (Survey Corps to Scouting Regiment, and Sasha's last name to Braus), but it flows a bit better this way I think.

There was a tweet last week (I think) from Studio Wit about hiring animators.
Let's hope this means that they are fully aware of all the bloopers in episode 4.
Thankfully, it was still quality enough to get the fandom excited. In terms of generating interest, I think episode 4 is on par with episode 1.
Right now though, I consider 4 as my least favorite, not because of the mistakes but mainly because of all the missing scenes from the manga.
