Being drunk excuses it a bit (although being drunk is also just as likely to lead people to just pass off or wander around being dicks).

One Night Stands and the like can be written off well (after all, not like they don't ever happen in real life too), but the trick is giving them a plot purpose at the end.

[up]Well, that would apply if both participants know the ins and outs of sexual intercourse enough that they are daring to experiment with stuff. Of course, to inject a little bit of reality, there has to be some nervousness.

I personally wouldn't know but I doubt it often happens.
From experience ... it is something that happens more often with practice. And somewhat realistically, depending on how the woman's muscles clench when she hits orgasm, it can easily be enough to tip the guy past the point of no return; whether that's close enough to count as "simultaneous" is up to you, I suppose, but I'd count it.

To go back to characters with canon relationships, for a moment, when the canon SO is turned into a wife-beating, mustache-twirling, happiness-sucking douchenozzle so either the author's preferred love interest can "save" their beloved, or so the canonically "taken" character can deliver a "Reason You Suck" Speech before switching to the preferred love interest.

I hate it when either partner in a romance plays an entirely passive role. Love is a two player game.
