After he does it the first time, his eyes change from partially outlined to fully outlined and his hair is different. This is consistent all the way to the end of the manga, during the part with Oob.
Some other media have portrayed it as a transformation he uses, either with his eyes not being outlined when he's resting or being fully outlined already and his transformation just powers him up. It looks like BoG and Super will have him "transform" with the only difference being his bang coming back.
It's possible that he's not transforming but instead just powering up like many other characters did at many points in the series.
Of course, there's still the bang and eye thing but shut up.

I'm sure he had that from the start, giving the point of the form. I think his situation would be similar to any base Saiyan, where they just freely adjust the power they're using on the fly.
I mean we see in the Boo arc that he's not running at full power all the time (he doesn't use his full power until right before he actually hits Boo) and I think he's effectively just like a base Saiyan - he can modulate his power, but not through transformation.

Regarding Ultimate Gohan
If "other media" is referring to the video games, I hasten to point out that the games have learned their lesson on this and have started depicting Ultimate Gohan as a separate character rather than a transformation.
They began doing this around the same time they started weaning off transformations like "Potential Unlocked" or "Fuse with Nail" that were never actually transformations.

Currently side-by-side liveblogging Digimon Adventure, sub vs dub.Xenoverse also brought back Potential Unlocked as a transformation, then confused the issue by having the old Kaioshin teach it to the player but making it mechanically identical to the version that Krillin and Gohan get from Guru and not the version Ultimate Gohan has.

But yea, Gohan probably uses his power like Kaioken and only in short bursts.
A lazy millennial who's good at what he does.I might be wrong on this, but I don't think Gohan's ever demonstrated the ability to do that.
Goku flaring up his power in short bursts when and where he needed it was a character-specific thing that highlighted his martial discipline and mastery over his power and set him apart from other characters. By the time he fought Frieza, he was even able to shock Krillin and Gohan when Piccolo had to inform them that he was doing that with the Kaioken - they had assumed he had a chance against Frieza because he hasn't fired up the Kaioken, but Piccolo informed them that he's been using it all this time. It's a Goku thing, frequently commented on by antagonists and protagonists alike.
Gohan tends to go all-out. He gets his power from pure rage and thus fights like a berserker, only restraining himself out of pacifism or sadism when he's achieved a form far beyond what his adversary is capable of. I don't think he's ever demonstrated the intense mental discipline that lets Goku so quickly and effortlessly manipulate his battle power in rapid bursts.

I know that, but does he power up to get that strong or is he just constantly that strong 24/7?
Like, I'm watching the Buu Saga and Old Kai told him to try to transform into a Super Saiyan, and he got all of his full power, but does he need to do that to get his full power before a fight or was a one-time thing?
