Non canonical sources seem to really like the word 'Ultimate'. Like how Gogeta SSJ 4 is 'The Ultimate Fusion', and stuff like that.
The 'Form' doesn't really have a name 'cause it's not really an ascended form or anything of the sort. Gohan doesn't have to do anything special to access that power and it's always available once unlocked. 'Ultimate' or 'Mystic' Gohan, however you want to call him... is just Gohan.
He's still less badass than he was when he was 11, btw.

Ultimate is the most recent term used, but he's also been called "Chou Gohan" (Chou means Super, but it's different to Super Saiyan because that's usually written in English), or "Strongest/Mightiest Warrior Gohan" (based on what Elder Kaioshin said).
Super Saiyan 3 Goku did indeed lose to Pure Boo because of the drawbacks of a living body using Super Saiyan 3. Which means he easily could have beaten Pure Boo at the time with Super Saiyan 3, he just needed to kill himself first then immediately be revived on the spot and use Super Saiyan 3 without drawbacks, since they're in the Kaioshin realm.
The Dragon Balls sort of needed to become as versatile as they did to avoid Unfortunate Implications of the mass resurrections. Being able to resurrect people multiple times only ever matters for Kuririn and Chaozu individually, but it also means that the mass resurrections aren't going to leave large numbers of people who already died in previous arcs still dead. It winds up being more for the sake of the civilians than anything else.
He was physically 10. And he wasn't very badass, he had two modes during the Cell Games: pacifist who forgot his character development, and cocky sadist who made the situation worse.

To be honest, there's not really much difference between physically 16 and physically 17.
I mean, most people wouldn't look much different at that point, so it's kinda moot to argue it.

The fighting games. For some insane reason, I just really, really like how Goku sounds when he's fighting. While the high pitched voice should be annoying... it just isn't when he's fighting. But I still vastly prefer his latin voice, both in and out of fights.
Also, lots of people can vary greatly on their apparent age depending on how they're standing, their clothes and the expression on their faces, which explains why Gohan can sometimes manage to look his actual age. That, and even subtle differences such as the sharpness of the lines and the angles of his face can make huge differences.
As for Goku looking like a little kid when he shouldn't... well, he is an alien. That might also explain Gohan, as well. Saiyan biology works in very odd ways, given that they don't actually become old, even though their lifespan is apparently no longer than a human's. (I'll always run with the headcanon of "No Saiyan has ever died of old age. When they became old and weak, another killed them in battle because that's the only dignified way to die for a Saiyan. So we have no idea how long they could actually live if given the chance to grow old, but it's somewhere in the ballpark of a human lifetime.")

Yes, he was originally 12... then 8 months passed and the RRA arc happened... then 3 years passed and the 22nd Budokai happened... where he had the same appearance as when he was 12. At 15-16.
The first time he appears to look like an adult is when he's 18, at the 23rd Budokai.

That's what Goku does.
It's also the most copied trait in fanfiction. Goku doesn't need a reason or motivation to train as hard as he can as best as he can. He does it simply because of the joy of improving himself and facing even greater challenges every time.

He doesn't really age, so much as he transformed from his Child form to his Adult form one day while training with God. I choose to believe it's a Saiyan thing.
