well it's funny because the protagonist of Iron Blooded Orphans deliberately engineered situations where the enemy wouldn't be able to fire without risking blue on blue.
He engaged at close range since he was out numbered, but since it was point blank (I can hit them with my sword distance), the enemy couldn't fire without risking what ever enemy the protag was currently fighting.
and then there is the already mentioned using a retreating group of enemy tanks as a shield

Its mostly aircraft on personal or artillery on personnel, people dont wear IFF tags like vehicles have now.
The airplanes just see dots on the ground, they can not make out what there shooting at, so if there are now friendlies in the area they were told to attack guess what happens?
Artillery don't even have visuals at all, they just have math, so they require damn good coordination through the radio and any mistakes can be fatal.
Infinatry on Infantry is uncommon since soilders do not identify based on uniform or camo, but rather what gun the other guy is using, since each gun sounds and looks very distinctive, this is why they are told to never pick up an enemy weapon under any circumstance.
Vehicle on Vehicle is also rare due to IFF tags, but does occasionally happen due to using outdated IFF codes.
Drone on infantry is also exceedingly rare, as drones are the only aircraft capable of seeing exactly what they are shooting at due to having cameras that actually zoom.
Basically the problem comes from mixing people and vehicles, the people can identify vehicles, but it is much harder for the vehicles to identify the people.
Plus what Lemon said is a big component for the infantry on infantry ones.
