My theory is that they'll replace that part of Born Again with whatever happened to Karen during her studiously untalked-about Dark and Troubled Past.

Sounds possible.
I mean, I know they won't be able to use Captain America for it. Shame, really, because he was one of the best parts of Born Again.
Also, sidenote, but apparently Karen's death in the Born Again comic is her last appearance ever in comics. So Frank Miller more or less turned her into a failed porn star drug addict who sold out her ex boyfriend to get a fix, and then got killed for it. Fuck you, good sir.

Yeah, it seemed clear to me that Fisk was going to figure it out on his own. But Karen's killing of Wesley is probably going to come back, too, and cause Fisk to want revenge on her.
Karen didn't die in "Born Again" — the story actually ends with Matt and Karen together. No, Karen was killed by Kevin Smith a decade or so later during the "Guardian Devil" story.

Personally, if I were to reveal Daredevil's identity, I'd do something like this

(Mind you, I haven't actually read that much of Mark Waid's run, but this scene would be amazing.
Oh God! Natural light!Hnm. Interesting. Daniel is considered an orphan when whomever takes him into K'un-Lun. In the comics I guess you can consider him an orphan though Lei Kung, the Thunderer isn't played up to being a parental figure. His relationship to K'un-Lun as not only a training ground but a newly found home for him doesn't have much story to it.
Another difference. Wendell, Heather, and Meachum take a plane towards K'un Lun rather than walk. Heather dies from it crashing in a storm instead of being eaten from wolves.
Daniel comes back to America more confused than ever, clearly dressed as if he has no sense of life outside K'un-Lun. In the comics, he blended pretty well in the rest of society without much effort.
My only speculation. Although not confirmed, I say he gets placed into the medical facility because he talked about K'un Lun after getting into trouble, a place that sounds like it came out of Mortal Kombat.
Question. Was Ninja ever casted. I forget. Because if so then we have Khan.

We actually already had Mike Murdock — when Matt wouldn't tell Claire his name at first, she called him "Mike."
And we probably won't get Karen the Drug-Addicted Porn Star, because that happened when Karen went out to Los Angeles to become a movie star and the Karen in the show doesn't appear to ever want to do that. She already has a second job right now as a reporter anyway.

To be fair, if she moves to the Punisher it would both make Daredevil less crowded and let us get rid of the dull, lifeless romance subplot.
Plus I might actually watch Punisher. Jon Berenthal did an excellent job, but I still have zero interest in seeing a 13 episode show that's mostly about Frank Castle.
But, if they're still only doing 2-3 shows a year that means there's going to be a pretty large gap between Daredevil seasons, not to mention all of the other shows; I could see them juggling actors around so more of them have steady work and don't make big commitments to like, an NBC show or something. I could easily see a few actors being regulars on more than one show.
