Its not so ridiculous that a wormhole would affect Tony more than any of his villains.
They were all things within his context. Even if they came close to killing him, they are all things he can understand and overcome.
The trip through the wormhole nearly killed him and acted as kind of a total perspective vortex for him.

616 Mags wouldn't try to murder his own children over them not being on board with his plans.
Existential dread is fine and all. But would it have hurt to give Tony a personal nemesis that by him/herself could leave half that kind of lingering itch on Tony? Tony has treated all of his previous villains as not even footnotes after their downfall. We've never seen Tony go, "I'll never forget what that bastard did to me/Pepper/Happy/etc." for any of his rogues. Yeah, it got a bit personal with The fake Mandarin after Happy nearly bit it. But that was gone soon after in the same movie. We need more MCU villains who actually can get under the hero's skin and stay there even when they're long dead.

I honestly don't think that would be as interesting, at least for Tony.
And honestly, that sort of villain seems kind of played out.

But if the villain in question is the person's ARCH enemy, then shouldn't he/she kind of go above and beyond the usual song-and-dance most of the Rogues Gallery puts the hero through and really f**k with him/her in some cerebral way (confidence, beliefs/worldview, etc.) ?

Well, considering how all the main villains die at the end of Iron Man movies, I don't know if they can do a good job of that.
Also, I'd prefer to avoid Stuffed in the Fridge.

Like I said, the villain in question doesn't necessarily have to remain alive in order leave a lasting mark on our hero.
...AND no one has to be Stuffed in the Fridge either. Sometimes, just making the hero question his own beliefs and values can be enough.

Oh God! Natural light!I find it interesting that Vanko was actively trying to be the sort of villain Nervmeister described, only, you know, he wasn't very effective at that aspect, probably due to the convoluted nature of the movie and the greater personal threat Palladium poisoning had on Tony.
I swear, that film tried to do too much and did too little with all of it.
The Blog The ArtOf course, it would be dark as hell and Marvel would probably never want Iron Man to be seen as an alcoholic, but still.
They sort of did, they just did a shitty job of it.
Iron Man 2 starts out like it's going to be Demon in a Bottle but then suddenly uses a curve ball to avoid anything really being Tony's fault.

Bane managed to give Batman quite a bit of psychological trouble simply by making him overtax himself and then beating him to near death, no H-lectures or fridge surprises required (unless you're referring to the TDKR version). The ultimate damage was not to Bruce's spine but his confidence. Something like that. So a mostly physical threat can be a good option too, especially if the hero in question prides him/herself on his abilities.

Ha ha! So Palladium and wormhole. Two things with no agency of their own have destabilized Tony worse than the very individuals out for his blood/misery..........and alcoholism. Real shame too. Because I could see a glimmer of potential in Vanko's characterization too. I mean, if his lines are fondly quotable among us, then there was something there that could've really taken off. But alas.

Tony privatized world peace. He's the Invincible Iron Man. He's more than ready for people to be out for his blood. It's something he can handle.
What he can't handle are things that can kill him without ever giving him the chance to fight back. Things he can't just shoot missiles into until they go down.
