The trinkets are a new kind of passive but some have only graphical effects (goat hoof = bigger tears some times but always same damage) to some that are OP (gives you a random mushroom effect every room).
The new final boss fight makes me feel sad for Isaac and the new ending I do not understand it completly.

Well you take damage and a chest appears, it can take 1 or more hearts of damage.
An easy rule is never go to that room or the curse room (the one with spikes ata the door) with a 1 heart or less of life because you will almost surely die unless you can fly in which case you just take half a heart of damage when leaving the curse room.
Yeah and Samason is quite afected by it. Samson is a strange character, his range and tears are low and his attack I think is a little less than Isaac but is good in rooms with multiple enemies due to his starting item. The complain I have is figth against peep or monstro or any boss that do not spawns enemies take a lot with his base damage but the ones with enemies like the duke of flies goes really fast.
Well, whole post will be spoiler to everyone who hasn't played game at all, so I'll only put spoiler tags(well, except for few specific spoilers) on part that details DLC.
But yeah
this is theory in its basics. In short Isaac in the end decided to hide in chest, that chest which we get ending items from.
Ok, so everyone have to remember one thing about the game. The fact that most things in the game are drawn on paper by Isaac. So most of the game is imagination by Isaac. Even parts that aren't that aren't on paper, like mom opening door in first ending.
First ending has Isaac draw himself happy ending, but since its not real it doesn't solve any problem. So mom is still trying to open door and Isaac gets struck out of his happy imagining by fear of her actually opening door.(which doesn't happen yet at least)
Endings in which Isaac pulls out stuff from chest are varied. In first one hes still pretty happy since he imagined new character to 'solve' is problems, but rest of them vary like hanging one is because Isaac basically wants to die at that point and money glass thing and Dr. Fetus are for boredom, wafer and quarter are something he really found in the chest and nail turning him into demon is foreshadowing. Anyway, most important part of them is mom's hand dragging Isaac to chest and Isaac finding ??? in chest. It Lives(aka, Isaac as fetus) replaced Mom's heart because at that point Isaac basically starts to wish he never had born.
So after that desperation point, Isaac starts thinking that in order for him to not die, since beating mom in his imagination doesn't help, he should beat evil itself to clear his sins. Leading to Sheol. So after Isaac has beaten Satan, he discovers that chest he found items from again. But it is empty. Scene shows all of Isaac's characters(excluding ??? and including demon Isaac) opening chest. No matter what character Isaac plays, no matter whether he used power of evil, chest is still empty. So Isaac decided to hide in chest.
??? is different from other playable characters(and reason why he doesn't appear in Sheol ending) as where they are Isaac wearing wigs and clothes and naming himself after Bible characters, ??? is Isaac imagining himself as dead. Between level nightmare shows Isaac suffocating in chest, and DLC adds item "Fate" which is ???'s head.(its blue in similar shade to ???)
DLC Features Isaac imagining himself alternative for trying to defeat Satan. Instead of trying to kill lord of sin, he should try to reach redemption from heaven. Leading to Cathedral. At end of Cathedral Isaac discovers himself, crying. Crying Isaac doesn't really fight back, he just sees light and gains wings and flies to heaven. Isaac sees himself as demon beating part of himself that was free of sin who got into heaven. After that we cut to Isaac reading bible(which is where he got his inspiration for his characters), watching at mirror, seeing himself as demon and finally resigning to his fate to die in the chest. Abused Isaac believes himself truly to be sinner, after all more useful power ups in his imagination are from Satan.
So yeah, in short(again) endings imply that in the end Isaac decided to go into the chest, ??? is him dead. Of course, nothing has happened yet as Isaac can't continue imagining while dead, but it seems that Isaac believes his fate to be dying in the chest.
So yeah, rather sad ending unless new dlc in future change it to happier one or something

Well, you can complete the game without getting to Devil Room, so Demon Isaac is just Isaac who had used Devil Room to get powers.
Really, since my theory is about game being imagination of Isaac, I'm pretty sure that Isaac believes his mom about him being full of sin since he imagines getting power from Devil to defeat his mom/Devil himself. Which is why in Wrath of Lamb ending demon Isaac is on mirror

Yeah the bloody lust bandana is quite powerfull, I had a run with Samson with three or four damage up efects and after two or three enemies the rest died really fast.
Speaking of Samson I am pretty sure that you unlock something by killing the devil with him but can't tell because the pop up are screwed and don't show up
The fetus in a jar is better, I would like to try it with the spacce bar item that lets you explode bombs manually.
