They don’t have to refuse the order, they can just chose to understand “attack Canada” to mean “draw up battle plans for a possible military strike on Canada in 12 months time, get the plans sorted and possible outcomes determined and have me review them in 6 months if I remember to ask for them. Oh and don’t start working on this until I’ve submitted the order in writing to you, your direct superior and four other relevant figures in the military chain of command”.
And yeah Canada is part of NATO, so any attack there would trigger NATO, which I think would mean that the US was legally attacking itself.

UCMJ only ever stipulates that personnel are required to follow lawful orders issued to them. The Army at least also has the Laws of Land Warfare, which lays out a requirement to disobey an order issued in direct violation of either domestic or international law. In practice this usually only applies to blatantly unlawful orders and usually not edge cases, but the situation we're discussing is a blatantly unlawful order.
War with Canada isn't even worth thinking about. It's simply not going to happen, you'd have to rewrite history for it to be in any way plausible.

The sleazy New York Democrats, and their now disgraced (and run out of town) A.G. Eric Schneiderman, are doing everything they can to sue me on a foundation that took in $18,800,000 and gave out to charity more money than it took in, $19,200,000. I won’t settle this case!......Schneiderman, who ran the Clinton campaign in New York, never had the guts to bring this ridiculous case, which lingered in their office for almost 2 years. Now he resigned his office in disgrace, and his disciples brought it when we would not settle.

For the record having a plan to invade an ally is actually a good idea even if you intend on remaining allies with them. Nations tend to be conquered or destabilize.
Let's say some other country invades Canada. We can adapt our plans to invade Canada to liberate them, too.
From the opposite perspective: Let's say (very hypothetically) the Southern States try to secede again. It would be in Canada's interests to help, and in the US's interests to accept said help. And if Canada had plans laying around to invade the US, they would be very useful here.

Few new updates.
1) The report on Comey's actions during the Clinton probe is out. It found that he did deviate from procedure at times (he didn't coordinate with DOJ superiors enough) but absolved him of political bias in either direction. It also concluded that Clinton did not evade charges thanks to political bias on anyone's part.

Like there are quite a few examples of Trump being worryingly authoritarian, and returning a salute isn't really one of them.
