That's how things would progress if they were operating by some semblance of rationality.
Trump has no semblance of rationality. When he does use nukes, be the target foreign or domestic, it's likely that it will be so sudden and so utterly pointlessly petty that it will shock even his most hardcore supporters.
Either that or it'll be in response to a long-running problem that he got tired of seeing on the news. He'll think of it as Cutting the Knot, not wiping out millions of lives.

This site lists off every member of Congress. You can add / and your state's abbreviations to get the page for your own reps and senators. I managed three of my five congresspeople. Apparently my zip code is split among more than one district or something and I'm not getting any results giving me some +4 numbers bs, so I can't even email two of the representatives. Three people is still not bad. If you want to contact your state governments with letters of your own, there's the one stop shop for their names and page links.
Well reducing any significant city in any state will swing it red. That's part of why Georgia had the county unit system for so long and why it retain vestige of rural control in its legislature. (It was "Southern Democrats" then, but their intellectual descendants are the modern Republican backbone). To this day, suburban sprawl stays what would otherwise be an overwhelming urban constituency.
Also his supporters ere looking for a strongman to punish "the other." The sudden and fiery disappearance of a major city may just as likely elicit cheers from his base as revulsion. Sympathy does not work when you've been labelled the enemy.

Beyond the fact that the most likely way nuclear weapons would be deployed against us (or anyone, really) is via ballistic missile (much faster and much smaller and harder to spot on the way down than a bomber, plus they can be crammed into submarines), I've long since accepted that if a nuclear exchange happens, I'm gonna fry, and I only hope I get vaporized by the initial blast, rather than burning to death or suffering from the radioactive effects of the weapon.
If it helps, provided that you aren't in a super-major city and/or near major military installations, you're probably safe from the effects of a retaliatory strike by anyone other than Russia, as the other major nuclear powers (UK, France, and China) only have about 200-300 warheads (India and Pakistan each have about 100-130 nukes, North Korea has about 10, and Israel (assuming they do in fact possess nukes) is a bit of an unknown, with estimates ranging from 80 to 400) many of which would be devoted to high-priority tagets like DC, STRATCOM, Cheyenne Mountain, and other major military facilities.
I live in a village not far outside a smallish city that's seen better days, butted right up against a wealthier one. As far as the nukes themselves, I'm safe. Fallout? I'm not sure if there's enough hills between the targets and me to stop that shit.
Eh, maybe I'll turn into a ghoul and have a real long lifespan. I already look like that Chris guy from New Vegas who thought he was a ghoul.
I live in California, which would be one of the biggest targets in a nuclear strike, but I live in the suburbs of Trabuco Canyon, not one of the big cities. Most of the media I watch, read and play is Japanese, though lack of Net Neutrality would cause problems with that since they are foreign (Should I back up pictures I like on Pixiv to my laptop?), and I talk with my friend on Deviantart (Which would also probably get affected by Net Neutrality) everyday.

Gold. Silver. Anything precious. They can be converted to whatever currency is best when this is all done. Avoid diamonds. Those are a fucking trap. They're not valuable, they're common, and no self respecting jewelry store ever gives you what you paid for them, let alone a profit. Emeralds, rubies, sapphires, sure. But diamonds? Wave them off or strip them off the bands and keep the gold and silver instead.
ETA: Eh, what I had in mind could have lead to pilarisation I suppose, but that wasn't the intent. It was more just a way for us to strike back in a language the republicans already knew without making enemies of the lower classes we're trying to help protect. I doubt it would work anyway. The gap is exaggerated to hell and back by those ninety nine percenters but not nearly enough for us to actually make up the difference for a select few who could fight the system. We'd wind up seeing our efforts coopted and broken the same way the Tea Party was.

The signs would be conventional conflict first. It could easily start with a Tweet but we'd likely get a demand for an apology, an attempted federalisation of a state's national guard to bring the offending politicians in by force, the attempted use of regular US military forces to bring the offender in by force and after that the deceleration of a state to be in rebellion followed by an attempted nuclear strike on that state's capital.
