If they die, they are replaced by their running mate (VP Pence for Trump, Senator Harris for Biden), and a new VP will be selected by the Senate. Its too late to replace the actual ticket/nominate a back up running mate.
Incapacitation could lead to the 25th Amendment being invoked at least on the short term, giving the VP the President's power until he recovers.

Politics is the skilled use of blunt objects.That was essentially Bolsonaro's play when he got (and subsequently recovered from) COVID.
"All you Fascists bound to lose."Sadly, you can be as cautious as you want, but if everybody around you isn't cautious, there's no guarantee you'll be okay. I worry about the outcome of this situation. People can cheer poetic justice if an anti-masker gets COVID, but there's always a risk of collateral damage, sadly. Not just for Biden but for anybody in that environment.
I do some cleanup and then I enjoy shows you probably think are cringe.I've read that after the White House learned about Hope's symptoms Trump flew to New Jersey, attended a fundraiser and held a speech. No idea who else in his administration was exposed to him in that time, but at the very least there's a real risk he's spread the virus to other people who probably have had contact with even more people.
Knowing trump's policies, including among much of his staff, we can expect many more positive tests in the coming hours and days. It has the potential for wildfire.
I don't think trump is faking it. He wouldn't fake a virus he keeps denying is a problem, and the US president getting it sends the message that ANYONE can get it. Like, it counters too many narratives on the right for it to be worth it.
He'll definitely downplay the severity because he downplays everything about the virus.

Unless he dies, of course. Let's get real, he may have access to the best medical treatment anyone in the world can possibly have, but he's still an unealthy old man in an age bracket that the virus kills more often than not. His survival is not guaranteed.

Do not obey in advance.Herman Cain died of COVID and his twitter keeps ranting about it being no big deal. Trump will be on his deathbed saying "it's no big deal, it's a fake virus", and if he dies people will just claim "it's not covid that killed him, it was an assassination by the satanic vampire cabal"
