I don't listen to a lot of soundtrack music honestly, but off the top of my head I'd say the score to Twin Peaks.
Easy question. The Matrix, Final Fantasy IX and Gurren Lagann each have soundtracks I consider among the best music ever.
Clockwork Orange, Pirates of the Carribbean, Fargo (and Carter Burwell generally for example the score of In Bruges), anything by Danny Elfman (= most Tim Burton movies), Kill Bill and generally Quentin Tarantino's taste in music though there will always be some bad choices among them, The Boat That Rocked, Noein, Oldboy (and generally music composed by Yeong-Wook Jo for those Park Chan-wook movies), Das Boot was neat, too.

Teh Poledouris is the shit when it comes to a soundtrack that works as a cohesive whole instead of just one or two tracks that rule but a whole lotta filler that sucks.

Garden State. I'm not particularly fond of the film itself, but it has a superb soundtrack.
Haibane Renmei. Surprise surprise, it's the same guy who composed the soundtrack for Shadow Of The Colossus.
And yeah, what that other guy said about Cowboy Bebop.
I mean, really, there's tons of stuff I could dig up, but I'm feeling lazy today, so I'm just mentioning the first examples that spring to mind.
Umineko No Naku Koro Ni (visual novel, of course). Always will be, until When They Cry V tops it.

Game soundtracks, I'm only really familiar with two items: the first two Fallout games - collected recently in the free Vault Archives compilation, not perfectly but it did the job since it wasn't compiled anywhere before that - and Assassins Creed II. Fucking hell the music in that game. For example, I just can't get tired of the following leitmotif, even though it's all over the soundtrack's first half:

the fact that Morricone's brand of simplistic epicness and driving western soundtracks hasn't been mentioned makes for a really sad Juan.

Anime: Cowboy Bebop, Evangelion in general and Rah Xephon (Only Anime opening I can actually tolerate)

, etc.), all injected with Hamauzu's signature piano prowess and vocal skills.
That's not even getting into the massive production values of the soundtrack itself. Almost all orchestral tracks have some kind of live performance in them; sometimes they're solo, but there are also plenty of full orchestral sections recorded. The synth quality is also top-notch, and the natural way it blends with the live instruments makes the two nearly indistinguishable. It's pretty amazing.
So, yes. Final Fantasy XIII's soundtrack gives me a gigantic musical boner.
I love the Lord Of The Rings soundtrack, and doubt that I would have liked the films as much if not for it. Other favorites are Conan The Barbarian, Back To The Future, and the Dune soundtrack (a movie doesn't have to be good to have a good soundtrack).
When it comes to video games, I have a soft spot for the Baldurs Gate stuff and I love Mark Morgan's Planescape Torment soundtrack. Warcraft II as well.
