dual destinies also had the dissasembly chair. not exactly a pleasant thought- the whole using it on a human thing. and its brought up several times quite explicitly, and at least one bad ending gave me the impression that the reason none of the hostages were ever seen again is because of that damn chair
i think that game also has a ton more blood than any of the others.

Rest easy, SSB 4 won't be M-rated. Not even the most graphic game starring Mario reached there.

the only thing of note i know about the ESRB is that they gave heart of darkness and its 40 or so ways to kill the child main character an E for everyone rating. presumedly because despite the breaking of bones, crushings, fallings, tearing aparts, dissintegrations, drownings, burnings, eaten, disembowlings and other such ways for andy to die, theres no blood.
there are sound effects though.
