In Golden Sun, my Djinn were always on Standby. Even if I wasn't going to summon anything, my mind thought that changing classes=bad. Even if it was matching Djinn with their elements, nope, they were never set. Therefore my characters were always weak and I could not fathom why.
Of course nowadays, I'm the complete opposite.

I was racing Hilary and due to sheer luck, I managed to get ahead of him, and I thought I had enough time to go and change my clothes.
He then sped past me, and I never beat him since.

Also, back in the days of the original Civilization: I tried to irrigate every square before founding my first (or any) city. Every square. Took me a while to figure out that was a bad idea.
On the other hand, there was this one ridiculously awesome game where I never actually built a city, instead using my Settlers to pop huts and get mercenaries to conquer everyone else's. Never finished it, though.

In Legend of Zelda: Link to the Past, I never thought to PUSH THE DOOR blocking the secret entrance to the sancuary and NEVER GOT PASSED getting the princess out of the jail cell. I am so glad I got the re-release for the GBA.
In Super Mario Kart, I never let go of the acceleration, also, I lapped people and got pissed at my SNES because I was wondering how they passed me.
In Pokemon, I would sneak past people who wanted to have a battle so I would get through the game as soon as possible. This led to me being 20 levels behind the Elite Four and I would have to restart my game. Then, I would still sneak past them.
I'd lag behind in Sonic Advance 2 because I figured you were supposed to destroy every single robot in the level. Note that I was 6 at the time. Needless to say, I never made it past the first zone.

There aren't really many situations where there's much reason to let go of the accelerator in Mario Kart, so that I find completely understandable.
Meanwhile, since Mario Kart was my introduction to racing games, years later I'd play Initial D: Arcade Stage and also never let go of the accelerator. I wasted several dollars before I realized that the brake pedal is actually useful.
Some time after that, I played Wangan Midnight: Maximum Tune and proceeded to brake too much.

Extra 1: Poochy Ain't StupidI used to spend literally hours in 3D platformers (notably Super Mario Sunshine and various Spyro The Dragon installments) just dicking around the game's 'hub' areas - climbing on roofs, swimming, trying to jump to unreachable platforms and perform game-breakers...
...no wonder I never beat any games as a younger kid...
