They can see the swirly eyes if they can detect the enchantment with their Sense Motive.

Push the button, if you dare. 🖲️I assume what happened is that originally both the real and fake passages matched the entrance, and then over the many years of nobody figuring out the trap, the real passage was disused, and the entrance and the fake passage experienced equal wear and discolouration.
ERROR: The current state of the world is unacceptable. Save anyway? YES/NONo, the unused part of the fake dungeon is the same color as the used part.
The entrance was simply recolored to match the fake dungeon in order to keep up the illusion.

By the way, looking back at this page, is there a mistake with the smear on the floor? In the panel where Roy is peeking into the end of the tunnel, half of the smear is visible on the tunnel side of the portal. But that's the fake tunnel, so the smear shouldn't be there.

That’s because the smear is only present in the “real” tunnel, the one the Order is in that’s connected to the door. In that panel Blackwing is in the “fake” tunnel; the smear isn’t there with him because that tunnel is somewhere else. In the panel where Roy pokes his head into the dead end, the smear shouldn’t be on either side of the line because it’s showing the fake tunnel on both sides, as indicated by Blackwing in the background. To only show it on Blackwing’s side of the line is an art error.
I’m thinking that now that the Order has been drawn away from their planned ambush, Redcloak is going to notice the half-smear and get clued in to the deception that way. It’s a pretty big tell-tale.
Yeah, I hadn't thought of it before all this discussion about the smear (which is actually more of a scorch mark). But yeah, what is visibly half of a scorch mark on the floor does seem like a clear giveaway that shenanigans are afoot if Redcloak happens to make the necessary Spot check to notice it.
