  • Best Known for the Fanservice: A good majority of the film's positive reviews tend to point that 1) Hayden Christensen finally gets to act properly, and 2) he gets shirtless a lot in this movie.
  • Catharsis Factor: After seeing Clayton so helpless and surrounded by so many cruel people, it's immensely satisfying seeing all of the corrupt surgeons and Sam get taken down and arrested by the police. Double points for the initially incompetent Dr. Lupin actually being the Token Good Teammate, who reveals to Lilith that her son is still alive while also helping ensure the cops catch the corrupt doctors.
  • Nausea Fuel: The surgery itself - already shown in gruesome detail - is made ten times worse by the fact that Clay is still awake.
  • Nightmare Fuel:
    • Would you want to watch this movie the night before any kind of surgery? Even a root canal?
    • The moments in which it slowly dawns on Clay that he's still aware of everything are absolutely terrifying.
  • Paranoia Fuel: How well do you know the people in your life, and for that matter, how moralistic if not competent are your doctors, especially those who are to perform life-saving surgery on you?
  • The Woobie:
    • Clay becomes one over the course of the film. He falls head over heels for Sam, grows close to Jack who saved his life and believes that he's going to get a second chance of life with a new heart, only to discover practically everything in his life was a lie and he ends up dying a cruel and painful death.
    • Lilith as well, possibly to an even bigger degree: she is fiercely protective of her only child and only wants what's best for him. She tries to warn Clay about his friend and girlfriend, neither of whom she trusts, and then loses him, which drives her to commit suicide in order to give him her heart. And it comes out that she killed her husband and Clay's father in self-defense years earlier due to him being a drug-addicted abuser who tried to attack him as well in one of his rages.
