- Accidental Aesop: Smoking kills. The cancer that John, a heavy smoker, ends up being diagnosed with is small cell carcinoma of the lungs, a cancer that is usually associated with smoking.
- Alternate Character Interpretation: Johnny's tale about the man whose death Carmine Sr. signed off on once his earning potential had evaporated is suspiciously similar to Johnny's own altercation with Ralph, only with exaggerated circumstances and the ending Johnny wanted. Hey John, did poor dishonored "Jimmy" even really exist?
- Harsher in Hindsight: Sydney Pollack played Dr. Feldman, an imprisoned oncologist who befriends Johnny Sack and often offers second opinions with respect to Sack's medical treatment. Pollack himself passed away from cancer on May 26, 2008.
- Jerkass Woobie: For all the evil that Johnny Sack did in his life, it's hard not to feel for him and his family as his inevitable death by cancer closes in on him.
