- The Scrappy: Despite being one of the "main three" heroines that has an extended Good Ending sequence, a lot of players are not big fans of Nagisa due to her Clingy Jealous Girl tendencies turning into outright stalker behavior and making Kaori more and more unhinged to the point where she fears for her life and yelps in fright just hearing her phone ring. It's hard to say if she redeems herself in her Good Ending, but her Bad Ending is one of the darkest due to her kidnapping Kaori, keeping her chained up and drugged in an unknown location, and the implication that Kaori soon dies as a result.
- Tear Jerker: Elements of the story itself due to Kaori's self-loathing or depression due to feeling inadequate, the reactions characters have when a patient dies, or some of the various bad endings.
- Sayuri's irreparably damaged relationship with her mother, which also causes her to reject Kaori's peace offer and rip her heartfelt letter in two in front of her in several routes.
- Sayuri's bad end results in her dying as a result of her body rejecting her bone marrow transplant. Kaori and her spend their last day together at the sea and then Kaori carries her into the ocean to lay her to rest.
