Teresa was not killed at the start of the pilot episode.
The first scenes in the pilot episode show Theresa arriving by helicopter with her entourage. She proceeds to tell the viewers how rich and powerful she is - then takes a sniper shot in the chest and falls to the floor where she appears to be bleeding out. Or is she? Right through the episodes so far she's suggesting she is not who and what she appears. Its quite possible "being killed by a sniper" is an escape route by which she can leave the Narco world without being hunted down and killed.
  • CONFIRMED. Teresa did have an escape route and it turns out that some elements of her plan were setup years in advance. There was a complication or two, but ultimately it went off without a hitch with even Devon being convinced.

Teresa is sending visions of herself to the past as she's dieing.
To put a bit of the supernatural into things, Teresa, as the Queen, is sending out her ghost into the past somehow to advise herself during key moments where a major decision needs to be made and/or needing general comfort during a particularly bleak moment. But how and why does the Queen know when exactly is the right moment to appear? It is because it all happened to her before, with those moments where the Queen appears were times when Teresa would have ended up dead if she made the wrong decision or action, and would have most likely did without the Queen's intervention. Teresa is basically applying rogue-like mechanics to her life, her own soul as the upgrades/strategy guide to help herself every time she died up to a certain point. The Queen getting shot at the beginning of the first episode is essentially the end of another failed run......
