  • Cowboy BeBop at His Computer: During the August 10th 2021 XBOX showcase, the game was reported to be a Japanese game developed by Type-Moon. In the latter's case, there was even audio of the Korean voice cast speaking.
  • Defictionalization: HamHam PangPang, the bread store mentioned by Roland in the game, was turned into an actual cafe by Project Moon in Suwon, South Korea. It is a themed cafe for this game and Lobotomy Corporation, with the store selling goodies and merchandise from the two games.
  • Development Gag: According to Kim Jiyeon in the Lobotomy Corporation artbook, Big Bird's original concept was that it was followed by smaller birds that resembled it, which would grow up into more big birds. During Big Bird's abnormality boss fight, it is followed by two smaller birds that resemble the ones in the comic Jiyeon made.
  • Meme Acknowledgment: The achievement for customizing a Librarian is called "Your Very Own Nugget!", referencing the Fan Nickname used since Lobotomy Corporation to refer to the playable non-Sephirah characters.
  • Nerf: Distorted Yan yoyoed with this to hell and back. On release, his fight was rather easy, so several of his pages, not including Distorted Blade, were buffed. The developer Jihoon Kim proceeded to fight him, proceeded to lose, and then immediately hit him with the Nerf bat - and included Distorted Blade this time around. However, only the enemy version of the card was hit, which is why the player version has a larger roll note .
  • Refitted for Sequel:
    • The Jester of Nihil was originally a planned Abnormality for Lobotomy Corporation named "The Jester's Drift", who was supposed to spawn when something happened with all three Magical Girl Abnormalities, similar to the Black Forest Abnormalities and Apocalypse Bird. Here, it's been refitted as the final abnormality on Tiphereth's floor.
    • The second fight of Hokma's floor, The Price of Silence, was present in early versions of Lobotomy Corporation, but was removed in the final product, it's abilities being converted to Hokma's Meltdown.
    • Nosferatu was a planned Abnormality for Lobotomy Corporation, but never saw the light of day. Here, he's the fourth fight of Gebura's floor.
    • The Silent Girl, another Lobotomy Corporation no-show, also finally appears as Carmen's last phase.
    • The Silent Orchestra had the ability to spread a Hate Plague, making agents fight eachother in the Legacy version of Lobotomy Corporation, which was taken out during its update. This ability makes a return in the Abnormality page 'Fervent Adoration', which makes enemies targeting random (possibly making them attack their own allies).
    • Big Bird's minions, the Eyeball Chicks, were an earlier concept that was scrapped before Lobotomy Corporation was released.

  • What Could Have Been:
    • Before the system of battling Abnormalities to unlock their pages was implemented, various Abnormality Pages were added which were never used. These Abnormalities are:
      • Floor of General Works: Mirror of Adjustment.
      • Floor of History: Beauty and the Beast, the Dreaming Current and the Shelter from the 27th of March.
      • Floor of Technological Sciences: 1.76 MHz, Schadenfreude, Behavior Adjustment, You Must Be Happy, We Can Change Anything and Dimensional Refraction Variant.
      • Floor of Literature: Old Lady and Meat Lantern.
      • Floor of Art: The Little Prince, Rudolta of the Sleigh and Theresia.
    • Early in the game's development, Project Moon hinted that one of the points that would be explored in the story would be what exactly happened to Agents hit by Execution Bullets in Lobotomy Corporation. This point was never explored, with the director explaining the team could not find a way to fit it into the game's narrative.
    • Do ProjectMoon Dream Of Auto Battle Simulation Game? reveals that Library of Ruina was originally going to have automatic combat.
    • The artbook revealed that there had been plans for Boris to undergo Distortion and become a boss fight in early development, though it was changed due to being noted as too graphic. The same explanation was used to explain why a planned CG of Roland's arm and leg being ripped off in the prologue was scrapped.
      • According to Project Moon director Kim Ji Hoon in the 2021 Project Moon Q&A video, if this was to happen, it should be imagined as "a symbolic manifestation of a [hydraulic] press. It is a menacing contraption that crushes everything within".
  • Tuckerization: In the February 19th, 2021 update, the new General Invitation enemy - Dong-hwan the Grade 1 Fixer - was named after character artist Kim Donghwan.
  • Word of God:
    • According to an interview with Project Moon, if the Pianist were to be classified as an Abnormality, he would be either a high-tier WAW or a low-tier ALEPH.
    • Project Moon's 2021 New Year Q&A revealed that San's little brother, who was hinted at in the Zwei pre-battle cutscene, is actually Bada from the Cane office.
