J.D. decides to take a break from narrating. Dr. Cox is more than happy to share his thoughts about his day, especially when the professional relationship with his therapist sours.

Tropes in this episode

  • A Day in the Limelight: This is the first of several episodes where different characters take over narrating from J.D., in this case, Dr. Cox.
  • Big Damn Heroes: Dr. Cox unwittingly saves J.D. from the janitor when opening up the stopped elevator at just the right time.
  • Character Development: Cox learns to open up a bit and let other people into his life, Eliot learns to be a little bit less insecure and be willing to date a man in a predominately female profession, and Carla realizes how and why she's been sabotaging her relationship with Turk.
  • Failed a Spot Check: Doug, twice. Cox asks him to find a chart that's obvious in its usual location at the end of the bed, then asks him to find the soda he was holding for him, when Cox is now holding it.
  • Life Saving Misfortune: While Dr. Kelso is mad at Dr. Cox for giving an honest evaluation, he's also grateful because high blood pressure can kill without treatment.
  • Jerkass Has a Point: When Paul tells Kelso that Eliot's embarrassed of him and he doesn't know why. Kelso's reply is "It's because you're doing a woman's job, son.". Yes, Kelso's a sexist piece of crap, but he's right. That sexism is why Eliot's embarrassed.
  • Never Heard That One Before: It's revealed that Dr. Cox thinks hard about what new girl's name to call J.D. J.D. politely tells him he used the one for this episode the week before. "Dammit!"
  • No, You: Kelso tells Zeltzer to shut up, and he quietly mutters "You shut up.".
  • Oblivious to His Own Description: Carla tries to deliver a long "The Reason You Suck" Speech to Eliot about why she's having trouble with Paul, but ends up describing her own relationship and why she's having trouble saying yes to Turk's marriage proposal. This leaves Eliot incredibly confused by the end of it.
    Eliot: Carla, I... just met the guy.
  • "The Reason You Suck" Speech: Dr. Cox's therapist lays into him for coming to basically bitch about his life and refuse to change. Therapy is all about being willing to listen, but Dr. Cox won't listen to the one person paid to care about his emotional well-being. Dr. Cox later admits that the guy is right.
    • JD gives a quieter one when he shows up to tell him that he's tired of all of his shit and has switched to another attending.
  • Taunting the Unconscious: Dr. Cox performs a physical on Dr. Kelso and discovers he has high blood pressure, which could have led to his death if not for Cox. When Kelso thanks him for it, Cox resists the urge to rub it in his face. However, Cox still feels the need to be a jerk to someone, so he insults a comatose patient.
