Elena is safe in East Adrougne, but she's not done yet- she wants to find out what the plague actually is so she can get started on curing it. During her stay in West Adrougne, she was able to take some good plague samples, but she can't do anything without her Distillator, which the Mourners confiscated. She wants you to go take it back. The sewer tunnel has been found and blocked off, so you need to go a different route- climbing a rope over the wall, distracting the Mourner guards with pigeons.

Once there, you sneak into the Mourner headquarters by poisoning their food and pretending to be a nurse there to help them, then killing a Mourner for a key to the cage Elena's stuff is in (Don't worry, we're pretty sure he had it coming). Once Elena has her stuff, she uses liquid honey, ethenea, and sulphuric bromide to get the sample to react with the touch paper, and change its color. The results of the test... nothing. The touch paper hasn't changed at all.

Elena is confused by this result and asks you to get her mentor, Guidor, to do the test to confirm that she's done it right. She hands you a plague sample, ethanea, honey, and sulphuric bromide and tells you to go to the chemist at Rimmington to get some touch paper.

Word of somebody carrying plague samples has spread, and you shouldn't tell the chemist about it unless you want to trek back to Elena for another sample. However, if you just say that you want to meet up with Guidor for some chemistry things on Elena's behalf, he'll be accomodating. You can't get through to the fenced-off section of Varrock where Guidor lives with all of the reagents in your inventory, so he'll tell you to give one vial each to his three assistants, Chancey, Hops, and Da Vinci, and tell them to meet you at the inn near Guidor's house.

The assistants each must be given a specific vial, because they'll lose the other two. Chancey will gamble away the ethenea and the sulphuric bromide, because they're valuable, but he can't do this with the liquid honey, which is worthless. Hops will drink the ethenea and the liquid honey, but he isn't stupid enough to try to drink sulphuric bromide, which is very poisonous. Da Vinci will want to use the reagents in his artwork, but will be stymied by the ethenea, which is colorless.

Meet up with the assistants at Varrock and get the reagents back, then dress up as a priest to get through Guidor's wife, who otherwise won't allow visitors. Guidor will perform the same test as Elena, and get the same result: The touch paper doesn't change, and so there is nothing wrong with the sample at all.

The plague that has devastated West Adrougne is fake.

You return to Adrougne and tell Elena, then confront King Lathas, who quarantined West Adrougne. He admits that the plague is fake, but says that it's to protect the people from his brother King Tyras, who has been corrupted by a strange being known as the Dark Lord. He lets you move freely from East to West Adrougne, and tells you that he may need your services in the future.
