Chimney takes the lead of the 118 while Bobby is suspended. Eddie discovers that he and Shannon have very different ideas to where their future leads. A distressing call causes Maddie to reconsider her own future at the dispatch centre.

First call for the 118 and Sgt. Grant involves a man who won the lottery, but the money apparently drove him to attempt suicide because it ruined his life, causing one woman who thought she had the lucky winning ticket to be so horrified at what could happen to her as well to tear it up so as not to meet the same fate. Also, pending Bobby's suspension until the investigation into St. Paul is completed, Chimney is serving as acting Fire Captain/Battalion Chief, meaning his duties as a paramedic are suspended while commanding officer of the 118. This doesn't work out well for the 118, and Buck visits Bobby, pleading for him to come back because of how Chimney is handling the 118 in Bobby's absence, soon followed by Hen, and then Eddie, followed by Athena's kids regarding Bobby becoming their step-father, and finally Chimney, wanting out of being the temporary Fire Captain.

The next call for the 118 is to the local chocolate factory, where a schoolteacher that had been off his chocolate obsession for some time ends up falling into a vat of dark chocolate upon being addicted to it again after tasting it, but thanks to some ingenuity from Chimney with administering some cocoa butter to help loosen the chocolate's viscosity, they are able to pull the teacher out, though Hen suspects Chimney called Bobby for help to complete the rescue.

At Parker Center, Maddie responds to a 9-1-1 call about a girl's younger brother contemplating suicide and she tries to help until the girl pulls up to her house to see all the LAPD cruisers there and hangs up, leading Maddie worried as to if she made it in time to save her brother.

Elsewhere, a couple are about to head out on a dinner date when the doorbell rings. When the wife answers it, a car drives off, while a package is left on the porch. Believing it to be from her husband, she shakes it, and it suddenly explodes; a mail bomb.

Athena, the LAPD Bomb Squad, and the 118 are summoned to the scene, and despite his status as captain, Chimney tends to the victim immediately while the Bomb Squad checks for more explosives. After the house is cleared, the wife is rushed to the hospital, but it leaves Athena and the 118 suspicious for who could've done it.

That night, Eddie and Shannon are having dinner together as they discuss their future and things with Christopher, but despite Shannon confessing that she's not pregnant with another child and explains everything to Eddie, she swears that she wants to prove herself a good mother and wife, but decides it might be best if she and Eddie get a divorce.

Maddie is speaking with her superior from the 9-1-1 Call Center, and as they talk, all the victims that Maddie helped appear to thank her, including the sister who hung up after fearing her brother had taken his own life, revealing that she and the LAPD made it in time and saved his life, and Gloria, the previous dispatcher who had been a bad dispatcher and was fired for it, but Maddie helped turn her life around.

Meanwhile, the 118 arrive to where someone drove into a crosswalk, and find to their horror that Shannon was among the victims, and worse still, critically-wounded, possibly fatally. Eddie is permitted to say his final goodbyes to Shannon before she is intubated and flatlines as they head for the hospital. Despite their efforts, Shannon is declared DOA (Dead On Arrival) at the hospital, and Eddie is given her clothes she was wearing at her time of death to take home as he gives the grim news to the rest of the 118. Even Bobby arrives to console him over Shannon's death, while Chimney feels horrible being unable to save Shannon's life when they return to the firehouse.

Hen comes to visit Chimney later to console him over believing he's to blame for Shannon's death, and reassure him that his temporary position as Fire Captain is not a curse, but a blessing, and he has her support as his partner and fellow paramedic.

Meanwhile, Bobby meets up with Athena, and as they prepare to head home, they see a news report about another mail bomb going off, causing them to realize they have a serial bomber on the loose.
