After seeing Retsuko sing, Hyodo proposes that Retsuko become the lead vocalist, demoting Manaka in the process.

- Oh, Crap!: Retsuko has this three times. The first time is when she sees Hyodo watching her after her Metal Scream performance in the karaoke; the second time when Hyodo chooses her as the new lead of the OTM Girls as consequence of the karaoke incident; and finally when he decides to use the new Death Metal demo in the concert to push her to sing.
- Older Than She Looks: Manaka reveals that despite being an idol (as Retsuko tries to justify that one of the reason why she refused Hyodo’s offer was for no longer having the age to do it), she’s actually 26 years old. Not even Hidari and Migi know about that until she openly points out.
- Title Drop:
- Of the series as a whole. After Retsuko uses her Metal Scream in the OTM Girls concert, Hyodo baptizes her idol role as Aggretsuko.
- The episode title is a reference for a metaphor that Hyodo uses to describe the fact that if they want to be famous they need to use the surprise effect (a.k.a. Retsuko’s screaming).
