- The Scarlet King is the embodiment of malice and cruelty in the SCP multiverse, and directly or indirectly the source of much of its chaos and corruption. Born as Khahrahk, he was a weak, but godlike being who was the first sapient being in the Darkness Below and quickly driven to hate all creation, and thus sought to destroy it. Starting off by devouring his siblings, Khahrahk went on to subjugate the Darkness Below, killing and enslaving gods and mortals across multiple dimensions. His influence on Earth includes ruling as the Daevite's ultimate god of their bloody theocracy, and the creation of James Anderson's factory. Becoming the Scarlet King, Khahrahk raped the goddess Sanna to death, before turning his seven daughters into his new brides. He would father monstrous children to serve in his armies; it is implied one such child is SCP-682. The Scarlet King caught the Foundation's attention when his cult was exposed and seven girls were found impregnated with his children; six of them died giving birth to monsters that would go on rampages, causing mass death and destruction.
- The Ambassador of Alagadda is the utterly sadistic representative of the Court of Alagadda. In Theology of a Snake, it's shown to have corrupted the previously human Hanged King and usurped his royal court, systematically devolving the kingdom into its twisted form. The Ambassador manipulated famed poet Christopher Marlowe into writing a self-evolving memetic virus in the form of SCP-701, a play that when performed would Mind Rape participants and audience alike into a homicidal frenzy, resulting in tens of thousands of massacres, human sacrifices and mass suicides for the last three hundred years, with some survivors left permanently psychotic or comatose. When the Foundation sent a MTF squad to investigate Alagadda through SCP-2264, the Ambassador forced the agents into torturing themselves, returning only one survivor as a warning to the Foundation, but not before making him see the face of the Hanged King.
- Francis "Fritz" Fritzwilliams is the enigmatic administrator of The Foundation. Respected by his underlings as the man responsible for defending humanity against eldritch horrors, Fritz is secretly a member of a cabal of immortal thrill seekers who perpetuate the Forever War between the factions in an endless strategy game for their own entertainment—he is personally responsible for the slaughter of up to a billion humans as part of the game. All the monsters that terrorize the world can be traced back to them, since they are fond of using SCP-5555 as a randomizer that changes the skillset of SCPs after each round, always giving them new ways to kill people. Believing empathy to be a weakness, Fritz tries to corrupt his pupil by ordering him to commit several atrocities, ultimately converting him into another immortal so he'll be forced to be his plaything for eternity.

- SCP-106, also known as "The Old Man", is a sadistic Walking Wasteland who loves kidnapping people, usually between the ages of 10 and 25 years, and bringing them in a Pocket Dimension where he plays a cruel cat and mouse game with no chance of escape. Once he captures the victim, instead of simply killing him, he tortures and mutilates him before returning the mangled corpse to the real world some days later. During Halloween, he escaped from his containment and started his murderous spree by killing a teenage girl, two young boys, and a mother. When an MTF agent was sent to recapture him, he tortured him to death by slowly taking chunks of flesh away from his body within a time span of four days until he finally succumbed to his injuries. While a literal monster, SCP-106 is shown to be fully sapient and his personality is one who sees torture, killing and mutilation as a game.
- SCP-953, also known as "The Polymorphic Humanoid", is a Korean Kumiho who makes a habit of devouring human livers. Before she was captured, the Kumiho would go on violent sprees to rip the livers from their owners and proved herself to be far more than just a predator. The Kumiho would proceed to draw out her victim's agony and viciously torture them to death, relishing in their pain. When she escapes, the Kumiho will inevitably begin her murders anew, including an attack on a furry convention that left many dead. She also murdered an agent's fiance and performed sexually indecent acts on the corpse right in front of the agent. Her worst crime, however, was telepathically forcing a mother to murder and cook her own child. On a Foundation investigation, she telepathically seduced the dispatched agents, made them cannibalize a rotten corpse, and killed two of the three agents, with one being killed mid-coitus, after first biting off his genitals.
- Bobble the Clown, the titular host of SCP-993, functions as The Corrupter to children. Using the power of television, Bobble would send viewers above the age of 10 into a trance, while children below that age were shown graphic acts of Bobble committing murder, cannibalism, torture, and other evil acts. Bobble instructs the children the ways to commit crimes, ingraining these ideas in their mind and causing them to grow up into psychopaths. Behind the scenes, Bobble describes himself as reaching into the heads of children and happily twisting them. It also reveals that his acts of violence might not be entirely fictional as he dismembers a Foundation doctor on TV and causes the doctor to disappear in real life. By his own admission, Bobble has been creating monsters for "quite some time"—apparently existing since the caveman days—and has even given his child victims instruction on how to release monsters from the SCP Foundation that could cause the end of humanity.
- Farmer rules over SCP-5045 as its tyrannical Killer Game Master, torturing hundreds of people for fun. Collecting the consciousness of players who download Goat VR, Farmer reshapes them into deformed goats to suffer as his pets for all of eternity, erasing the individuality of those who misbehave. When contacted by the lead researcher on his case, Farmer initially pretends to be misunderstood but quickly drops the act once the man sees through his façade, demanding that they stop deleting his online posts to torture more visitors and gleefully admitting that he enjoys hurting innocent human beings before finding a way out of the game and getting his hands on Site-301.
- SCP-6618-A, also known as "Papa", is the entity behind SCP-6618. Having kidnapped James Vane, "Papa" subjects him to severe psychological and physical torture under the guise of "disciplining" Vane, causing a slow regression in the latter's mental state. After Vane attempts to escape his abuser, "Papa" punishes him by repeatedly throwing him to the floor, heavily injuring his body, while also Gaslighting Vane into thinking "Papa" loves him. When the Foundation investigates "Papa's" cabin, they find boxes filled with other SCP-6618 instances and a pit containing at least several hundred corpses, Vane's among them. All of these corpses are wailing like infants even after their bodies have rotted away, with some of them being crushed to the point of near liquification and granularization, and are only able to cry for their Papa, who has already escaped the Foundation's grasp.

- James Anderson, from SCP-001:O5, is the head of The Factory. A secret follower of some unholy cult, he founded The Anderson Factory, which was publicly a massive worker's utopia, but was in truth a nightmarish hellhole. People were worked to death in inhuman conditions, injured workers were experimented into mad monstrosities, girls as young as eight were chained into "breeding pits" and subject to unspeakable horrors. Alongside normal products, the Factory produced horrible anomalous items that often do more harm to their users than good. Even after government agents raid the Factory and kill Anderson, Anderson, in an utterly inhuman and almost demonic form, made a deal with the founder of the Foundation, saving the Foundation from being defeated by the fair folk but reviving the Factory and forcing the Foundation to continuously feed it with people, ensuring that its terrible legacy lives on forever.
- Randolph Thaddeus Metzger, the cruel owner of SCP-748, follows in the steps of James Anderson. Making a deal with a mysterious entity known as The Investor, Metzger joins The Factory by sacrificing his entire family—his own children included—and servants to him. Metzger's factory is a dystopia where thousands of workers suffer on a daily basis before they are fed to the Crucible to be made into raw materials. Those who are spared from the Crucible are taken to be lobotomized, skinned, plasticized, and finally turned into zombified Cannon Fodder for The Factory—Metzger extends this fate to the staff of Site-68. Metzger even provides The Investor with weaponry when he begins planning a war, calling it a "most profitable venture". Allying himself with the rest of The Factory and launching dangerous products around the world, Metzger is willing to kill countless people to live in a society where only the producer and consumer exist.
- D-3826 was once a Serial Killer who used to do things the "old-fashioned way". After finding SCP-1034 on one of his "work tables", he began using it on his victims, which paralyzed their bodies, forced them to suture their own facial orifices, and trapped their souls in inside their bodies, leaving only their mouths open for them to scream in pain for an eternity. Aside from physical torture, it is implied that he took advantage of their situation and used their bodies as sex toys. When in the custody of the Foundation, he confesses that he enjoyed hearing the screams and begging of "his girls" before ultimately falling victim to SCP-1034 himself.
- Dr. Elliott Emerson, the merciless director of Site-13 in an Alternate Universe, is the man responsible for SCP-1730. Once an unpopular but ambitious member of Jack Bright's research team, Emerson is suspected to have framed Bright for the murder of a co-worker to get his promotion. As a Professional Butt-Kisser of the Foundation/Coalition union, Emerson ran Site-13 as his own "death camp" for fifteen years. Emerson experimented on every anomalous being that he could get his hands on; performing "invasive testing" on both animals and humans before going for vivisections; using the incinerator for "everything" or dumping the countless victims into a "body pit"; and eventually ordering the full-scale termination of the entire humanoid wing, his cruelty disgusting even this universe's version of Bobble the Clown himself. Deciding that he would use the Thresher and leave the containment breach for another universe to handle rather than face the consequences, Emerson activated the device and trapped everyone before they could even escape the facility. In Bobble's own words, Emerson was ultimately a sadist who got his jollies off watching pain, horror, and death.
- Dr. Rasmin Yelkov is the Mad Scientist behind the creation of SCP-1994. Contacted by his peer, Dr. Grigori, with the task of synthesizing human teeth, Dr. Yelkov's solution was disproportionately lethal. After meeting with a man who lives with the mystical teeth-growing SCP-478, Dr. Yelkov killed the man and captured the fairies when his purchase offer was rejected. Dr. Yelkov dissected the fairies and performed cruel experiments on human subjects, some of which are children, to develop a serum that grows rotten calcified masses to all over the body. When the growths became contagious, the uncaring Dr. Yelkov sees it as a minor setback and plans on continuing his research elsewhere, while still delivering the contagious, rot-covered corpses to Dr. Grigori and considering his lack of response—due to having been killed—to be a sign of success.
- Otari "Zver" Iosava, the crime lord behind SCP-2408, is the leader of The Hunter's Black Lodge, a Neo-Sarkic sect that also doubles as The Mafiya. Willing to do anything as long as it is profitable, Zver is involved in organ harvesting, Human Trafficking, and even selling SCP-610 as a bioweapon. A front for the Black Lodge is the Red Lanterns, a popular nightclub where women are forced into prostitution and some of the workers are carnivorous nymphs who feast on their clients. Zver distributes hyper-addictive drugs extracted from his brother's anomolous brain-dead body, the drugs not only mutate the users but also cause a horrific epidemic in Russia. Zver is also fond of hosting brutal Gladiator Games for the entertainment of influential followers of Sarkicism. Even his own allies are not safe from him, as Zver was not above butchering his more moderate Proto-Sarkic associates.
- Cornelius P. Bodfel III/Karcist Sulkisk, the occultist behind SCP-2480, was a High Priest in Sarkicism and the leader of Adytum's Wake, the oldest Neo-Sarkic secret society in North America. Bodfel owned a manor near a coastal town in Massachusetts and would kidnap its innocent citizens to be used as unwilling sacrifices in sadistic orgies, children included; Bodfel's victims would be raped, cannibalized and murdered by his prestigious acolytes, with Bodfel himself keeping journals and photographs of his crimes. When the Global Occult Coalition invaded his manor and killed him, the interruption of a ritual ended up turning the town into a gateway for eldritch monsters to slaughter humans, taking countless more lives even after his death.
- Dr. Madison Craggs, the jovial scientist behind SCP-3033, is a Psychopathic Womanchild with a severe case of Chronic Backstabbing Disorder. An American who defected to the Soviets during the Cold War before joining the Chaos Insurgency, she was the one who proposed the idea of the Mike units to Delta Command. Experimenting on hundreds of people with live brain surgeries to create a generation of Super Soldiers, Madison's Mikes are made to commit atrocities in name of the Chaos Insurgency while they are aware of everything. When given a particular target by Delta Command, Madison is excited to send her Mikes to kill hundreds of people at a GRU site simply because she wanted to get revenge on a man for having supposedly keyed her car in the past. After Madison was finally captured, she betrayed the Insurgency to live comfortably under Foundation custody, surviving an assassination attempt and vivisecting a desperate Mike for fun.
- "Enigma" is the pseudonym of the pick-up artist behind SCP-3512. Joining the Fifth Church to use its magic, Enigma creates living sculptures made of human fat that overtake his victims, turning the women into hollow living sex dolls incapable of objecting to his advances, with their initial consciousness occasionally screaming in terror before their complete cessation. Enigma shares his methods in his book The More You Know, where he instructs readers to stop thinking of women as people and murder their friends and family as sources of fat for the sculptures, sharing a pre-press copy with an associate who uses the process on nine women. When Foundation agents Rey and Cooper investigate his cavern hide-out behind the Sagrada Familia, Rey is lost and Cooper is made a victim of his. Enigma then rapes her before abandoning her in a cistern with at least eight other women, who are ground to be used as a source of human fat. Escaping consequences, Enigma plans to use his cavern's printing presses to further spread his methods.
- SCP 4231-A, or Lilly, is a powerful Type Green and the abusive girlfriend of Francis Wojcienchowski—later known as Dr. Alto Clef. Once a close friend of Francis, Lilly would become abusive towards him in their teenage years, with her abuse consisting of sexual assault, physical violence, and psychological abuse. Francis's abuse at the hands of his former friend would leave him severely traumatized to the point of him regularly having nightmares about his experiences and developing dissociative symptoms, which eventually culminated in Francis taking on a whole new identity and personality as "Alto Clef". After finding a copy of an esoteric book, Lilly becomes a cultist of the Scarlet King, killing cats to test her powers before kidnapping six girls, including at least one child, and subjecting them to ritualistic torture to allow the Scarlet King to impregnate them and offering herself as one of his brides. When the GOC intervenes, Lilly directly or indirectly causes the infamous Cornwall Incident, causing the deaths of 1,200 people by being boiled alive. Ultimately killed by Francis to save their daughter from her, Lily's actions caused the Scarlet King to haunt her universe, forcing the SCP Foundation to create the horrific Procedure 110-Montauk to prevent the arrival of the evil deity.
