Watch his expression when they try to explain - in a very roundabout way - the "mistake" that happened that afternoon, when his sister tried to have him committed. Elwood doesn't know anything went wrong. From his perspective, he was taken on a nice tour and offered a bath. But these two nice people are so concerned and guilty about an error they made right in this office ... yes, Elwood thinks they're confessing about having had sex while he was upstairs. This also explains his later conversation with Dr Sanderson:Dowd: Confidentially, considering what happened this afternoon, these flowers should really be coming from you.
