The 62nd successor to Hokuto Shinken and the half-brother of Ramon Kasumi. Originally an assassin that did as he pleased in Shanghai, Kenshiro upturned the balance of the city in favor of the Qing Bang Triads by slaughtering the malevolent Honghua Huì gang. Despite attempting to live a quiet life in Japan since, fate draws him back to Shanghai when news breaks out the Honghua Hui have regained power and slaughtered his best friend and lover, Pan Guang-Lin and Yu-Ling.

The leader of the Qing Bang and the best friend of Kenshiro. While Kenshiro was away from Shanghai, he was overthrown by the Honghua Hui and left to rot in confinement.

Guanglin's sister and Kenshirō's girlfriend. She was supposedly killed by Man Kuangyun when the Honghua Hui took over, but he simply erased her memories and sent her away, whereupon she eventually became a feared warrior known as Li Xiu-Bao that fought off Japanese imperialists.

Hokuto Sankaken

The North Dipper Three Family Fists (北斗三家拳) are three martial arts styles that branched off from Hokuto Shinken during the Three Kingdoms period and went on to develop their styles independently after the main Hokuto Shinken school was relocated to Japan. They are named after the Cao, Sun, and Liu families, which they were each sworn to protect and serve. The Liu Family Fist would later branch off from the other school and become the Hokuto Ryūken school from Fist of the North Star.

A flippant assassin of the Honghua Hui with a deep grudge against Kenshiro. He is a practitioner of Hokuto Sonkaken that mastered the forbidden art of "Pressure Point Displacement" (秘孔変位), allowing him to displace many of his pressure points to vastly increase his power at a severe cost to his lifespan. He kickstarts the plot by supposedly murdering Yuling and Guanglin, hoping the news draws Kenshiro back to China for a fight.

Qing Bang

Qīng Bāng (Chinese: 青幫; Japanese: 青幇), an influential criminal organization led by Pan Guanglin. They formerly controlled Shanghai's underworld through their operations primarily in the opium trade until they lost power after Kenshiro Kasumi left China in 1932.

Honghua Hui

A rival gang to the Qing Bang and the main antagonistic force in the series. As opposed to their rivals, the Honghua Hui has no ethics and values self-preservation over the common good. After overthrowing the Qing Bang, they incidentally spur Kenshiro back into action after supposedly murdering his lover and best friend.
