Dumah has overthrown Naomi as Ruler of Heaven. Having recruited Jack, she heralds a reign of terror in which humans are beaten into submission or forced to ascend to Heaven where Jack can transform them into angels, prompting Castiel to try to stop it. Meanwhile, Dean and Sam are arguing about the best way to bind Jack.

* BetterTheDevilYouKnow: This seems to be Castiel's attitude about Naomi. While she is complicated, her conviction is purely to prevent Heaven from collapsing, not making it the center of a dictatorship like Dumah does.
* CharacterDeath: Jack kills Dumah to end her brutal reign over Heaven and Earth.
* {{Cliffhanger}}: The episode ends with Jack breaking out of the Ma'lak box and confronting Dean, Sam, and Castiel.
* CreatingLifeIsBad: Other than personal concern, Castiel is mad at Dumah for telling Jack to create angels because of the way Jack does it, namely, by transforming human souls into graces, instead of creating them from whole cloth.
